摘要
目的:探讨自拟通闭益气汤联合腔内介入术治疗下肢深静脉血栓(deep venous thrombosis,DVT)的临床效果及作用机制。方法:本研究纳入2019年1月—2023年12月寿光市中医医院普外科收治的84例下肢DVT患者,采用随机数字表将其分为观察组与对照组,各42例。对照组采用腔内介入术治疗,包括血栓清除、溶栓、抗凝等;观察组在对照组基础上应用自拟通闭益气汤治疗。观察指标包括静脉临床严重程度评分(venous clinical severity score,VCSS)、血管通畅度、血清炎症因子水平及血栓后综合征(post-thrombotic syndrome,PTS)发生率。结果:与治疗前相比,治疗后两组VCSS均下降,观察组低于对照组,差异有统计学意义(P<0.05)。观察组血管通畅度优于对照组,差异有统计学意义(P<0.01)。与治疗前相比,治疗后两组血清IL-6、hs-CRP、TNF-α水平降低,观察组低于对照组,差异有统计学意义(P<0.05)。观察组PTS发生率低于对照组,差异有统计学意义(P<0.05)。结论:自拟通闭益气汤联合腔内介入术治疗下肢深静脉血栓可显著改善临床症状,改善血管通畅度,减轻炎症,降低PTS发生风险。
Objective:To investigate the clinical efficacy and mechanism of action of self-formulated Tongbi Yiqi Decoction combined with endovascular intervention in the treatment of lower extremity deep venous thrombosis(DVT).Method:A total of 84 patients with lower extremity DVT admitted to the General Surgery Department of Shouguang Traditional Chinese Medicine Hospital from January 2019 to December 2023 were included in this study.They were randomly divided into observation group and control group,with 42 cases in each group.The control group was treated with endovascular interventional procedures,including thrombus clearance,thrombolysis,anticoagulation,etc;the observation group was treated with the self-formulated Tongbi Yiqi Decoction on the basis of the control group.The observation indicators included venous clinical severity score(VCSS),vascular patency,levels of serum inflammatory factors and the incidence of post-thrombotic syndrome(PTS).Result:Compared with before treatment,the VCSS of both groups decreased after treatment,the score of the observation group was lower than that of the control group,the differences were statistically significant(P<0.05).The vascular patency rate of the observation group was higher than that of the control group,the difference was statistically significant(P<0.01).Compared with before treatment,the levels of serum IL-6,hs-CRP and TNF-αin both groups decreased after treatment,the levels in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).The incidence of PTS in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion:The combination of self-formulated Tongbi Yiqi Decoction and endovascular intervention significantly improves clinical outcomes in patients with lower extremity DVT,enhancing vascular patency,reducing inflammation,and lowering the risk of PTS.
