摘要
目的:观察针刺伏兔穴联合超声引导下腰神经根阻滞治疗腰椎间盘突出症(lumbar disc herniation,LDH)的临床疗效。方法:前瞻性纳入我院2023年3月至2024年6月期间收治的60例LDH患者,采用随机数字表法分为单一治疗组(超声引导下腰神经根阻滞治疗)和联合治疗组(采用针刺伏兔穴联合超声引导下腰神经根阻滞治疗),各30例。比较两组临床疗效、日本骨科协会(japanese orthopaedic association,JOA)、Oswestry功能障碍指数(oswestry disability index,ODI)、SF-36健康调查量表(mos 36-item short form health survey,SF-36)、疼痛视觉模拟(visual analogue scale,VAS)评分。结果:两组一般资料对比无差异(P>0.05)。联合治疗组的临床总有效率高于单一治疗组(P<0.05)。两组治疗3个月后、治疗6个月后ODI指数下降,JOA评分升高(P<0.05);联合治疗组治疗3个月后、治疗6个月后ODI指数低于单一治疗组,JOA评分高于单一治疗组(P<0.05)。两组治疗3个月后、治疗6个月后VAS评分下降,SF-36评分升高(P<0.05):联合治疗组治疗3个月后、治疗6个月后VAS评分低于单一治疗组,SF-36评分高于单一治疗组(P<0.05)。结论:针刺伏兔穴联合超声引导下腰神经根阻滞治疗LDH,可提高临床治疗效果,减轻疼痛程度,改善腰椎功能,提高生活质量。
Objective:To observe the clinical efficacy of acupuncture at Futu point combined with ultrasound-guided lumbar nerve root block on the treatment of lumbar disc herniation(LDH).Methods:A total of 60patients with LDH who were admitted to our hospital from March 2023 to June 2024 were prospectively included,and they were divided into single treatment group(ultrasound-guided lumbar nerve root block treatment)and combined treatment group(acupuncture at Futu point combined with ultrasound-guided lumbar nerve root block treatment)by random number table method,with 30 cases in each group.The clinical efficacy,japanese orthopaedic association(JOA),oswestry disability index(ODI),mos 36-item short form health survey(SF-36),visual analogue scale(VAS)were compare between the two groups.Result:There was no significant difference in the general data between the two groups(P>0.05).The total clinical effective rate of the combined treatment group was higher than that of the single treatment group(P<0.05).The ODI index decreased and the JOA score increased of both groups at 3 months and 6 months after treatment(P<0.05).The ODI index of the combined treatment group was lower than that of the single treatment group,and the JOA score was higher than that of the single treatment group(P<0.05).The VAS index decreased and the SF-36 score increased of both groups at 3 months and 6 months after treatment(P<0.05).The VAS score of the combined treatment group was lower than that of the single treatment group,and the SF-36 score was higher than that of the single treatment group(P<0.05).Conclusion:Acupuncture at the Futu point combined with ultrasound-guided lumbar nerve root block on the treatment of LDH,can improve the clinical efficacy,relieve pain,improve lumbar function,and enhance the quality of life.
