摘要
目的 探究不同根管充填方法对老年糖尿病(DM)伴慢性根尖周炎(CAP)根管治疗患者疼痛、咀嚼功能及炎症反应的影响。方法 选择96例于2023年3月至2024年3月在河南省第二人民医院进行根管治疗的DM伴CAP老年患者,通过随机数字表法分为A组、B组和C组,各32例。3组分别接受iRoot SP糊剂单尖法、AH-plus糊剂热牙胶垂直加压法和AH-plus糊剂冷牙胶侧压法进行根管充填,比较3组患者的疼痛程度、咀嚼功能、血清炎症因子指标和临床疗效。结果 3组治疗前视觉模拟评分法(VAS)量表得分、咬合力、咀嚼效率、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)差异无统计学意义(P>0.05)。治疗后VAS量表分值均降低,且A组VAS量表分值降低幅度更明显(P<0.05);治疗后咬合力和咀嚼效率均提高,且A组咬合力和咀嚼效率提升幅度更明显(P<0.05);治疗后IL-6、hs-CRP、TNF-α均降低,且A组降低幅度最显著(P<0.05);A、B、C组的临床疗效分别为98.18%、96.49%、84.62%(P<0.05)。结论 iRoot SP糊剂单尖法根管充填方式较其他两种根管充填方式能够更好地减轻DM伴CAP老年患者的疼痛程度,提高其咀嚼功能,减轻炎症反应,增强临床疗效。
Objective To explore the impacts of different root canal filling methods on pain,mastication function and inflammatory reaction in elderly patients with diabetes mellitus(DM)and chronic apical periodontitis(CAP)undergoing root canal therapy.Methods From March 2023 to March 2024,ninety-six elderly patients with DM and CAP who underwent root canal treatment at Second People’s Hospital of Henan Province were randomly separated into group A,group B and group C,with 32 cases in each group.The three groups were treated with root canal obturation using the iRoot SP paste single-cone method,the AH-plus paste thermoplasticized gutta-percha vertical compaction method,and the AH-plus paste cold gutta-percha lateral compaction method,respectively.The degree of pain,chewing function,serum inflammatory factor indicators,and clinical efficacy were compared between the two groups.Results There were no statistically significant differences in the visual analog scale(VAS)scores,masticatory force,chewing efficiency,interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),and high-sensitivity C-reactive protein(hs-CRP)among the three groups before treatment(P>0.05).After treatment,the VAS scores decreased in all groups,with a more pronounced reduction in group A(P<0.05).Masticatory force and chewing efficiency improved in all groups,with more significant improvements in group A(P<0.05).IL-6,hs-CRP,and TNF-αlevels decreased after treatment,with the most significant reduction observed in group A(P<0.05).The clinical efficacy rates for groups A,B,and C were 98.18%,96.49%,and 84.62%,respectively(P<0.05).Conclusion iRoot SP paste single pointed root canal filling method can better alleviate the pain level,improve chewing function,alleviate inflammation,and enhance clinical efficacy in elderly patients with DM and CAP compared to the other two root canal filling methods.
