摘要
目的 基于“肺-肠-菌轴”理论,阐释增液汤治疗功能性便秘(FC)的作用机制。方法 通过相关关键词检索文献,采用定性分析整合“肺-肠-菌轴”的病理关联、FC中肠道菌群失调的作用,以及增液汤对肠动力、肠道菌群及Cajal细胞的调控效应。结果 (1)“肺-肠-菌轴”失衡与FC的发生相关;(2)FC患者存在肠道菌群失调;(3)增液汤可通过多靶点调节该轴系,发挥治疗FC的作用。结论 增液汤治疗FC的机制在于通过“肺-肠-菌轴”介导的多环节协同作用,既调节肺肠神经-体液通路以改善肠道分泌与动力,又重塑肠道菌群微生态、修复肠间质细胞功能,形成“肺-肠-菌轴”的整体调控。本研究为中医“肺与大肠相表里”理论提供了现代生物学依据,也为FC的中药治疗提供了多靶点干预策略,助力中西医结合诊疗方案的优化。
Objective To elucidate the mechanism of action of Zengye Decoction in treating functional constipation(FC)based on the“lung-gut-microbiota axis”theory.Methods This study explored the mechanism of action of Zengye Decoction in treating FC based on the“lung-gut-microbiota axis”theory.Literature was retrieved using relevant keywords,and qualitative analysis was employed to integrate the pathological correlations of the“lung-gut-microbiota axis”,the role of gut microbiota dysbiosis in FC,and the regulatory effects of Zengye Decoction on intestinal motility,gut microbiota,and interstitial cells of Cajal(ICCs).Results The literature analysis revealed that:①Dysregulation of the“lung-gut-microbiota axis”is significantly associated with the pathogenesis of FC;②Gut microbiota dysbiosis is present in FC patients;③Zengye Decoction exerts therapeutic effects on FC through multi-target regulation of this axis.Conclusion The mechanism by which Zengye Decoction treats FC involves multi-link synergistic actions mediated via the“lung-gut-microbiota axis”.It not only modulates the neurohumoral pathways connecting the lung and intestine to improve intestinal secretion and motility but also remodels the gut microecology and restores ICCs function,thereby achieving holistic regulation of the“lung-gut-microbiota”axis.This study provides a modern biological basis for the TCM theory of the“exterior-interior relationship between the lung and large intestine”and offers a multi-target interventional strategy for the TCM treatment of FC,contributing to the optimization of integrated Chinese and Western medicine diagnostic and therapeutic approaches.
