摘要
目的 观察在初产妇助产护理中应用分娩球结合自由体位的效果及对减轻分娩疼痛、缩短总产程的作用。方法 按照随机数字表法将2020年6月-2022年6月天津和平新世纪妇儿医院收治的200例初产妇分为对照组(100例)和研究组(100例)。对照组应用常规护理干预,研究组在常规护理基础上应用分娩球结合自由体位干预。比较两组分娩方式、不良妊娠结局、产程时间、疼痛程度。结果 与对照组分娩方式相比,研究组自然分娩率更高,剖宫产率及阴道助产率更低(P<0.05);与对照组不良妊娠结局发生率相比,研究组不良妊娠结局发生率更低(P<0.05);与对照组产程时间相比,研究组第一产程、第二产程、总产程时间更短(P<0.05),而两组第三产程时间比较,差异无统计学意义(P>0.05);与对照组疼痛程度相比,研究组产妇潜伏期、活跃期、第二产程的VAS评分更低(P<0.05)。结论 分娩球结合自由体位在初产妇助产护理中的应用效果确切,能够提高自然分娩率,改善产妇不良妊娠结局,缩短总产程时间,减轻分娩疼痛。
Objective To assess the efficacy of birthing ball combined with upright positions in midwifery nursing of primipara and its effect on relieving labor pain and shortening the total stage of labor.Methods According to the random number table method,200 primiparas admitted to Tianjin Heping New Century Women′s and Children′s Hospital from June 2020 to June 2022 were divided into a control group(100 primiparas)and a study group(100 primiparas).The control group was received with routine nursing intervention,and the study group was received with birthing ball combined with upright positions on the basis of routine nursing.The delivery mode,adverse pregnancy outcome,stage of labor and pain degree(VAS score)were compared between the two groups.Results Compared with the control group,the natural delivery rate of the study group was higher,and the cesarean section rate and vaginal midwifery rate were lower(P<0.05).Compared with the incidence of adverse pregnancy outcomes in the control group,the incidence of adverse pregnancy outcomes in the study group was lower(P<0.05).Compared with the control group,the first stage of labor,the second stage of labor and the total stage of labor in the study group were shorter(P<0.05),while no significant difference was observed in the third stage of labor between the two groups(P>0.05).Compared with the pain degree of the control group,the VAS score of the latent period,active period and second stage of labor in the study group was lower(P<0.05).Conclusion The application effect of birthing ball combined with upright positions in midwifery nursing of primipara is exact,which can improve the natural delivery rate,improve the adverse pregnancy outcome,shorten the total stage of labor and reduce the pain of delivery.
