摘要
目的 探讨蠲痹痛方联合运动疗法治疗早期膝关节骨性关节炎(KOA)的效果,分析其对患者西安大略麦马斯特大学骨关节炎指数(WOMAC)评分及中医证候的影响,为临床治疗提供依据。方法 选取2020年10月-2021年10月武威市中医医院收治的82例早期KOA患者作为研究对象,采用随机数字表法分为对照组和观察组,每组41例。对照组口服氨基葡萄糖胶囊治疗,观察组口服蠲痹痛方联合运动疗法治疗。比较两组临床疗效、治疗前后中医证候积分(关节僵硬、活动受限、关节冷痛、关节肿胀)、WOMAC评分(疼痛、僵硬、生理功能)及血清炎症因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-10(IL-10)]水平。结果 观察组总有效率为92.68%,高于对照组的75.61%(P<0.05)。治疗前,两组中医证候各维度积分、WOMAC各维度评分及TNF-α、IL-10水平比较,差异无统计学意义(P>0.05);治疗后,观察组关节僵硬、活动受限、关节冷痛、关节肿胀积分均低于对照组(P<0.05),观察组WOMAC疼痛、僵硬、生理功能评分均低于对照组(P<0.05),观察组血清TNF-α水平低于对照组,IL-10水平高于对照组(P<0.05)。结论 蠲痹痛方联合运动疗法治疗早期KOA可提升临床疗效,改善患者中医证候及膝关节功能,调节炎症因子水平,具有较高的临床应用价值。
Objective To study the effect of Juanbi Tong formula combined with exercise therapy on early knee osteoarthritis(KOA),and to analyze its effect on the Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)score and TCM syndromes,so as to provide basis for clinical treatment.Methods A total of 82 patients with early KOA admitted to Wuwei Hospital of Traditional Chinese Medicine from October 2020 to October 2021 were selected as the research objects,and they were divided into control group and observation group by random number table method,with 41 patients in each group.The control group was treated with oral glucosamine capsules,and the observation group was treated with oral Juanbi Tong formula combined with exercise therapy.The clinical efficacy,TCM syndrome scores(joint stiffness,activity limitation,joint cold pain,joint swelling),WOMAC scores(pain,stiffness,physiological function)and serum inflammatory factors[tumor necrosis factor-α(TNF-α),interleukin-10(IL-10)]levels before and after treatment were compared between the two groups.Results The total effective rate of the observation group was 92.68%,which was higher than 75.61%of the control group(P<0.05).Before treatment,there were no significant differences in the scores of TCM syndromes,WOMAC scores and the levels of TNF-αand IL-10 between the two groups(P>0.05).After treatment,the scores of joint stiffness,limited activity,joint cold pain and joint swelling in the observation group were lower than those in the control group(P<0.05).The pain,stiffness and physiological function scores of WOMAC in the observation group were lower than those in the control group(P<0.05).The level of serum TNF-αin the observation group was lower than that in the control group,and the level of IL-10 was higher than that in the control group(P<0.05).Conclusion Juanbi Tong formula combined with exercise therapy in the treatment of early KOA can improve clinical efficacy,improve TCM syndromes and knee joint function,and regulate the level of inflammatory factors,which has high clinical application value.
