摘要
目的探讨闭合手法复位联合小夹板外固定术对桡骨远端骨折患者的治疗效果。方法于2024年9月-2025年8月选择我院70例桡骨远端骨折患者,按照随机数字表法分为对照组和观察组,各35例。对照组实施切开复位内固定治疗,观察组采用闭合手法复位小夹板固定治疗,对比两组临床疗效、骨折愈合时间、肢体肿胀缓解时间、Gartland-Werley评分、疼痛(VAS)评分、桡骨高度、掌倾角及并发症发生率。结果观察组治疗优良率(91.43%)与对照组(94.29%)对比,差异无统计学意义(P>0.05);观察组骨折愈合时间短于对照组,肢体肿胀缓解时间短于对照组(P<0.05);术后第1个月,两组VAS评分、Gartland-Werley评分均低于术前、术后第1天(P<0.05),且术后第1天观察组VAS评分低于对照组(P<0.05),但术后第1天两组Gartland-Werley评分对比,差异无统计学意义(P>0.05),术后第1个月两组Gartland-Werley评分、VAS评分对比,差异无统计学意义(P>0.05);术后第1天、第1个月两组桡骨高度、掌倾角均大于术前(P<0.05),术后第1天观察组桡骨高度、掌倾角均小于对照组(P<0.05),两组术后第1个月桡骨高度、掌倾角与术后第1天对比,差异无统计学意义(P>0.05),术后第1个月两组桡骨高度、掌倾角对比,差异无统计学意义(P>0.05);随访1个月,观察组并发症发生率低于对照组(P<0.05)。结论闭合手法复位小夹板固定治疗对桡骨远端骨折患者创伤小,利于骨折愈合,可减轻患者疼痛,快速缓解肢体肿胀,利于关节功能恢复,且可降低并发症发生率,具有一定的临床应用优势。
Objective To investigate the therapeutic effect of closed manual reduction combined with small splint external fixation on patients with distal radius fracture.Methods From September 2024 to August 2025,70 patients with distal radius fractures in our hospital were selected and divided into control group and observation group according to the random number table method,with 35 patients in each group.The control group was treated with open reduction and internal fixation,and the observation group was treated with closed manual reduction and small splint fixation.The clinical efficacy,fracture healing time,limb swelling relief time,Gartland-Werley score,pain(VAS)score,radius height,palmar tilt angle and complication rate were compared between the two groups.Results There was no significant difference in the excellent and good rate of treatment between the observation group(91.43%)and the control group(94.29%)(P>0.05).The fracture healing time of the observation group was shorter than that of the control group,and the relief time of limb swelling was shorter than that of the control group(P<0.05).On the first month after operation,the VAS score and Gartland-Werley score of the two groups were lower than those before operation and on the first day after operation(P<0.05),and the VAS score of the observation group on the first day after operation was lower than that of the control group(P<0.05),but there was no significant difference in Gartland-Werley score between the two groups on the first day after operation(P>0.05),meanwhile there was no significant difference in Gartland-Werley score and VAS score between the two groups on the first month after operation(P<0.05).The radial height and palmar tilt angle of the two groups on the first day and the first month after operation were higher than those before operation(P<0.05).The radial height and palmar tilt angle of the observation group on the first day after operation were lower than those of the control group(P<0.05).There was no significant difference in the radial height and palmar tilt angle on the first month after operation and on the first day after operation between the two groups(P>0.05).There was no significant difference in the radial height and palmar tilt angle between the two groups on the first month after operation(P>0.05).After 1 month of follow-up,the incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion Closed manipulative reduction and small splint fixation for the treatment of distal radius fractures has less trauma to patients,which is conducive to fracture healing,can reduce pain,quickly relieve limb swelling,facilitate joint function recovery,and can reduce the incidence of complications.It has certain clinical application advantages.
