摘要
目的:研究应用手法复位结合小夹板固定及祛瘀膏外敷,对于老年C1型Colles骨折患者的临床疗效观察。方法:选取2023年1月—2024年3月湖北省中西医结合医院骨科一病区收治的60例老年人C1型Colles骨折患者作为研究对象,根据不同的治疗方案,将研究对象随机分配为两组,即治疗组与对照组,每组包含30个病例。对照组在接受手法复位及小夹板固定后,外部再辅以普通无菌纱布进行包扎,治疗组给予手法复位小夹板固定后联合祛瘀膏外敷。治疗后开始观察并记录数据,比较两组治疗前后疼痛、肿胀程度,骨折愈合程度、炎症因子水平。结果:治疗后两组腕部肿胀程度、视觉模拟疼痛评分(VAS)评分、断端骨痂生长情况评价、炎症因子(C反应蛋白、白细胞介素-6)治疗后的平均水平相较于治疗前有所下降,且治疗组的降低程度显著大于对照组,差异有统计学意义(P<0.05)。结论:手法复位小夹板固定联合祛瘀膏外敷可显著缓解老年人C1型Colles骨折患者患肢疼痛、肿胀等症状,减轻炎症反应,促进骨折愈合。
Objective:To investigate the clinical efficacy of manual reduction combined with small splint fixation and external application of Quyu ointment in elderly patients with type C1 Colles fractures.Method:Sixty elderly patients with type C1 Colles fractures admitted to the First Department of Orthopedics of Hubei Hospital of Integrated Traditional Chinese and Western Medicine from January 2023 to March 2024 were selected as the research subjects.According to different treatment plans,the research subjects were randomly assigned to two groups,namely the treatment group and the control group,with 30 cases in each group.Patients in the control group received manual reduction and small splint fixation,and then were externally bandaged with ordinary sterile gauze.In the treatment group,manual reduction and small splint fixation were combined with external application of stasis removal ointment.After the treatment,the degree of pain and swelling,the degree of fracture healing,and the levels of inflammatory factors were compared between the two groups before and after the treatment.Result:After treatment,the average levels of wrist swelling degree,Visual Analogue Scale(VAS)score,evaluation of callus growth at the fracture end,and inflammatory factors(C-reactive protein,interleukin-6)in both groups decreased compared with those before treatment.Moreover,the reduction degree in the treatment group was significantly greater than that in the control group,and this difference was statistically significant(P<0.05).Conclusion:Manual reduction with small splint fixation combined with external application of stasis ointment can significantly relieve symptoms such as pain and swelling of the affected limb in elderly patients with type C1 Colles fractures,reduce inflammatory responses,and promote fracture healing.
