全方位智能静脉输液质控管理体系的构建与成效评估

摘要 目的 构建一套全方位智能静脉输液质控管理体系,并通过质控结果验证该体系的实践成效。方法 2020年1月—2024年12月,玉溪市人民医院通过一系列措施,致力于构建并持续完善全方位智能静脉输液质控管理体系。系统性收集2020年1月—2024年12月玉溪市人民医院的相关数据,数据源自医院信息系统及静脉用药集中调配中心的日常质控监测,具体指标为:静脉输液使用率、静脉输液规范使用率、静脉输液配置差错率以及新生儿肠外营养液配置误差率。同时整合了区域质控中心24家医疗机构的静脉输液使用率,以评估该体系的实践成效。结果 2020—2024年,玉溪市人民医院的住院患者静脉输液使用率分别为(89.99±1.11)%、(87.35±0.93)%、(85.53±1.12)%、(85.29±4.47)%、(82.90±1.85)%,住院患者静脉输液规范使用率分别为(84.00±1.21)%、(85.42±0.34)%、(87.08±1.52)%、(89.08±2.41)%、(96.33±1.24)%,静脉输液配置差错率分别为(0.006±0.003)%、(0.003±0.002)%、(0.002±0.001)%、(0.002±0.001)%、(0.003±0.002)%,各个年份间差异均有统计学意义(P<0.05)。其中,2022年静脉输液月平均使用率低于2021年,2021年静脉输液月平均使用率低于2020年,差异均有统计学意义(P<0.05),住院患者静脉输液使用率在2020—2022年逐年下降。2024年静脉输液月平均规范使用率高于2023年,2023年静脉输液月平均规范使用率高于2022年,差异均有统计学意义(P<0.05),住院患者静脉输液规范使用率在2022—2024年逐年升高。2024年新生儿肠外营养液配置误差率为(0.67±0.41)%,高于2023年的(0.44±0.36)%,差异无统计学意义(P > 0.05)。2024年质控中心各医疗机构住院患者静脉输液平均使用率为(84.21±13.05)%,低于2023年的(86.94±11.03)%,差异无统计学意义(P > 0.05)。结论 全方位智能静脉输液质控管理体系可降低住院患者静脉输液使用率和静脉输液配置差错率,提高住院患者静脉输液规范使用率。提示全方位智能静脉输液质控管理体系在静脉输液管理中切实可行。 Objective To construct a all-round intelligent intravenous infusion quality control management system,and verify the practical effect of the system through the quality control results.Methods From January 2020 to December 2024,People's Hospital of Yuxi City has been committed to building and continuously improving a all-round intelligent intravenous infusion quality control management system through a series of measures.The relevant data of People's Hospital of Yuxi City from January 2020 to December 2024 were systematically collected.The data were derived from the hospital's information system and the daily quality control monitoring of the centralized intravenous medication preparation center.The specific indicators were:the usage rate of intravenous infusion,the standardized usage rate of intravenous infusion,the error rate of intravenous infusion preparation,and the error rate of parenteral nutrition solution preparation for newborns.Meanwhile,the usage rates of intravenous infusion in 24 medical institutions of the regional quality control center were integrated to evaluate the practical effectiveness of the system.Results From 2020 to 2024,the usage rates of intravenous infusion among inpatients in People's Hospital of Yuxi City were(89.99±1.11)%,(87.35±0.93)%,(85.53±1.12)%,(85.29±4.47)%and(82.90±1.85)%,respectively.The standardized utilization rates of intravenous infusion for inpatients were(84.00±1.21)%,(85.42±0.34)%,(87.08±1.52)%,(89.08±2.41)%and(96.33±1.24)%,respectively.The error rates of intravenous infusion configuration were(0.006±0.003)%,(0.003±0.002)%,(0.002±0.001)%,(0.002±0.001)%and(0.003±0.002)%,respectively.The differences among each year were statistically significant(P<0.05).Among them,the average monthly usage rate of intravenous infusion in 2022 was lower than that in 2021,and the average monthly usage rate of intravenous infusion in 2021 was lower than that in 2020.The differences were statistically significant(P<0.05).The usage rate of intravenous infusion among inpatients decreased year by year from 2020 to 2022.The average monthly standardized usage rate of intravenous infusion in 2024 was higher than that in 2023,and the average monthly standardized usage rate of intravenous infusion in 2023 was higher than that in 2022.The differences were statistically significant(P<0.05).The standardized usage rate of intravenous infusion among inpatients increased year by year from 2022 to 2024.The error rate of parenteral nutrition solution configuration for newborns in 2024 was(0.67±0.41)%,which was higher than(0.44±0.36)%in 2023,but the difference was not statistically significant(P>0.05).In 2024,the average usage rate of intravenous infusion among inpatients in various medical institutions under the quality control center was(84.21±13.05)%,which was lower than(86.94±11.03)%in 2023,and the difference was not statistically significant(P>0.05).Conclusion All-round intelligent intravenous infusion quality control management system can reduce the utilization rate of intravenous infusion and the error rate of intravenous infusion configuration in inpatients,and improve the standard utilization rate of intravenous infusion in inpatients.It is suggested that all-round intelligent intravenous infusion quality control management system is feasible in intravenous infusion management.
