摘要
构音障碍是由发音器官神经肌肉器质性病变引发的言语运动控制障碍,以发音、共鸣及韵律异常为典型特征。脑卒中后构音障碍(post-stroke dysarthria,PSD)作为脑卒中后常见的获得性语言障碍,不仅严重损害患者的语言表达与交流能力,更对其心理健康及社会功能造成持续负面影响。随着脑卒中病理机制研究的不断深入及康复治疗技术的创新发展,PSD的临床诊疗已成为神经康复领域的重要研究方向。本文系统综述了近年来中西医领域在PSD发病机制、评估方法及康复治疗方面的研究进展,为优化临床诊疗方案、推动循证医学实践提供科学依据。中西医联合康复治疗方案在改善PSD患者言语功能方面具有显著协同效应,可有效提升康复效率与治疗效果。本研究为完善标准化评估流程、制定个性化康复方案提供了一定的理论基础与实践指导。
Dysarthria is a speech motor control disorder caused by organic lesions of the neuromuscular system of the speech organs,typically characterized by abnormalities in pronunciation,resonance,and prosody.Post-stroke dysarthria(PSD),as a common acquired language disorder after stroke,not only severely impairs patients'language expression and communication abilities but also has a persistent negative impact on their mental health and social functions.With the continuous deepening of research on the pathological mechanisms of stroke and the innovative development of rehabilitation treatment technologies,the clinical diagnosis and treatment of PSD have become an important research area in the field of neurological rehabilitation.To systematically review the research progress in the pathogenesis,evaluation methods,and rehabilitation treatments of PSD in the fields of traditional Chinese medicine and western medicine in recent years,so as to provide a scientific basis for optimizing diagnosis and treatment plans and promoting evidence-based medical practice.The combined rehabilitation treatment plan of traditional Chinese medicine and western medicine has a significant synergistic effect in improving the speech function of PSD patients,which can effectively enhance the rehabilitation efficiency and treatment effect.This study provides a solid theoretical basis and practical guidance for constructing a standardized evaluation process and formulating personalized rehabilitation plans.
