摘要
目的:探讨超声引导下星状神经节阻滞(SGB)联合推拿在混合型颈椎病中的应用效果。方法:回顾性分析2023年1月—2024年12月九江市第一人民医院收治的102例混合型颈椎病患者,按照治疗方法不同分为对照组(n=49,采用超声引导下SGB治疗)和联合组(n=53,采用超声引导下SGB联合推拿治疗)。对照组每隔3 d治疗1次,共治疗6次,联合组中的推拿治疗于每次超声引导下SGB治疗后的次日开展1次,共6次。比较两组临床疗效、颈椎疼痛评分(VAS)、颈椎功能障碍指数(NDI)评分及椎动脉血流动力学[收缩期峰值血流速度(Vp)、舒张末期血流速度(Vd)、平均血流速度(Vm)]。结果:联合组治疗后总有效率高于对照组(94.34%vs 79.59%)(P<0.05)。两组治疗后颈椎VAS评分、NDI评分均较治疗前下降,Vp、Vd、Vm均增快,且与对照组相比,联合组治疗后颈椎VAS评分、NDI评分低,Vp、Vd、Vm快(P<0.05)。结论:超声引导下SGB联合推拿治疗混合型颈椎病能提高临床效果,有效改善疼痛症状及颈椎功能,调节椎动脉血流动力学。
Objective:To explore the application effect of ultrasound-guided stellate ganglion block(SGB)combined with massage in mixed cervical spondylosis.Method:A retrospective analysis was conducted on 102 patients with mixed cervical spondylosis admitted to Jiujiang NO.1 People's Hospital from January 2023 to December 2024.They were divided into the control group(n=49,treated with ultrasound-guided SGB)and the combined group(n=53,treated with ultrasound-guided SGB combined with massage)according to different treatment methods.The control group was treated once every 3 days for a total of 6 times.In the combined group,massage treatment was carried out once the day after each ultrasound-guided SGB treatment for a total of 6 times.The clinical efficacy,cervical pain scores(VAS),neck disability index(NDI)scores and vertebral artery hemodynamics[systolic peak blood flow velocity(Vp),end-diastolic blood flow velocity(Vd),mean blood flow velocity(Vm)]of the two groups were compared.Result:The total effective rate of the combined group after treatment was higher than that of the control group(94.34%vs 79.59%)(P<0.05).After treatment,the cervical VAS scores and NDI scores in both groups decreased compared with those before treatment,Vp,Vd and Vm were increased,and compared with the control group,the cervical VAS scores and NDI scores in the combined group were lower after treatment,and Vp,Vd and Vm were faster(P<0.05).Conclusion:Ultrasound-guided SGB combined with massage in the treatment of mixed cervical spondylosis can improve the clinical effect,effectively alleviate pain symptoms and cervical spine function,and regulate the hemodynamics of the vertebral artery.
