摘要
目的:探讨高频重复经颅磁刺激联合常规康复训练对脑卒中后认知障碍伴吞咽障碍的影响。方法:选取沭阳县中医院2023年2月—2025年1月收治的125例脑卒中后认知障碍伴吞咽障碍患者,按抽签法将其分为两组,对照组(n=63)给予常规康复训练,观察组(n=62)开展高频重复经颅磁刺激联合常规康复训练,两组干预2周。比较干预前后两组认知功能、吞咽功能、生活质量、神经功能损伤程度、肢体功能及血清指标[血清视锥蛋白样蛋白1(VILIP-1)、神经元特异性烯醇化酶(NSE)、扣针蛋白5(Fibulin-5)、神经肽Y(NPY)]。结果:干预后观察组MoCA与SF-36各项评分,Fugl-Meyer肢体运动功能评定量表(FMA)评分高于对照组(P<0.05)。干预后观察组标准吞咽评估量表(SSA)与NIHSS评分低于对照组(P<0.05)。干预后观察组各血清指标低于对照组(P<0.05)。结论:脑卒中后认知障碍伴吞咽障碍患者行高频重复经颅磁刺激联合常规康复训练效果可改善患者生理机能,强化吞咽功能,改善病症对患者日常生活影响,缓解认知障碍,有助于患者尽早恢复健康。
Objective:To investigate the effects of high-frequency repetitive transcranial magnetic stimulation combined with conventional rehabilitation training on cognitive impairment and dysphagia after stroke.Method:A total of 125 patients with cognitive impairment and dysphagia after stroke admitted to Shuyang Hospital of Traditional Chinese Medicine from February 2023 to January 2025 were selected.The patients were divided into two groups by the lottery method.The control group(63 cases)received conventional rehabilitation training,while the observation group(62 cases)underwent high-frequency repetitive transcranial magnetic stimulation combined with conventional rehabilitation training.Both groups received interventions for 2 weeks.Cognitive function,swallowing function,quality of life,degree of neurological impairment,limb function,and serum indicators[serum cone proteinlike protein 1(VILIP-1),neuron-specific enolase(NSE),Fibulin-5,neuropeptide Y(NPY)]of the two groups before and after the intervention were compared.Result:After the intervention,the scores of MoCA and SF-36,as well as the score of Fugl-Meyer assessment of limb motor function(FMA)in the observation group were higher than those in the control group(P<0.05).After the intervention,the standard swallowing assessment(SSA)and NIHSS scores of the observation group were lower than those of the control group(P<0.05).After the intervention,the serum indicators of the observation group were lower than those of the control group(P<0.05).Conclusion:The effect of high-frequency repetitive transcranial magnetic stimulation combined with conventional rehabilitation training for patients with cognitive impairment and dysphagia after stroke can improve their physiological functions,strengthen their swallowing function,alleviate the impact of the disease on their daily life,relieve cognitive impairment,and help patients recover their health as soon as possible.
