外骨骼机器人辅助康复联合物理治疗在脑卒中患者下肢康复中的疗效探究

Efficacy of exoskeleton robot-assisted rehabilitation combined with physical therapy in lower limb rehabilitation of stroke patients

摘要 目的:对比分析外骨骼机器人辅助康复联合常规物理治疗与常规物理治疗在脑卒中患者下肢康复中的疗效。方法:选取2023年12月—2025年4月于南通大学第二附属医院·南通市第一人民医院收治的100例脑卒中患者作为研究对象,采用随机数字表法分为观察组(n=50,外骨骼机器人辅助康复联合常规物理治疗)和对照组(n=50,常规物理治疗)。比较两组患者在干预前和干预3周后的下肢运动功能【采用Fugl-Meyer下肢运动功能评定量表(FMA-LE)进行评估】、平衡能力【采用Berg平衡量表(BBS)评估】、日常生活活动能力【采用改良Barthel指数(MBI)、脑卒中影响量表(SIS)评估】、步行能力【采用6分钟步行试验(6MWT)评估,同时检测6MWT测试结束时患者HR、SpO_(2)、Borg自觉疲劳量表(RPE)】及康复治疗满意度。结果:干预3周后,两组患者的FMA-LE、BBS、MBI、SIS评分与6MWT结果、HR、RPE评分均较干预前提高(P<0.05),SpO_(2)均较干预前降低(P<0.05);且观察组患者的FMA-LE、BBS、MBI、SIS评分与6MWT结果、SpO_(2)均高于对照组(P<0.05),HR和RPE评分均低于对照组(P<0.05)。观察组患者的康复治疗满意率高于对照组(P<0.05)。结论:外骨骼机器人辅助康复联合常规物理治疗能够显著提升脑卒中患者下肢康复效果,有助于患者下肢运动功能恢复、平衡能力改善、日常生活活动能力提升、步行能力增强,患者康复治疗满意度高。 Objective:To compare the efficacy of exoskeleton robot-assisted rehabilitation combined with conventional physical therapy versus conventional physical therapy alone in lower limb rehabilitation of stroke patients.Methods:100 stroke patients who were treated at the Second Affiliated Hospital of Nantong University/Nantong First People’s Hospital from December 2023 to April 2025 were selected.They were randomly divided into the observation group(n=50,receiving exoskeleton robot-assisted rehabilitation combined with conventional physical therapy)and the control group(n=50,receiving conventional physical therapy).Lower limb motor function[Fugl-Meyer assessment of lower extremity(FMA-LE)],balance ability[Berg balance scale(BBS)],activities of daily living[modified Barthel index(MBI)and stroke impact scale(SIS)],walking ability[6-minute walk test(6MWT),heart rate(HR),blood oxygen saturation(SpO_(2)),and Borg rating of perceived exertion(RPE)measured at the end of the 6MWT],and rehabilitation satisfaction level were compared between the two groups before and after 3 weeks of intervention.Results:After 3 weeks of intervention,both groups showed significant improvements in FMA-LE,BBS,MBI,SIS,6MWT and RPE scores compared to those before intervention(P<0.05),while SpO_(2) levels decreased(P<0.05).The observation group had significantly higher FMA-LE,BBS,MBI,SIS,6MWT scores and SpO_(2) levels than the control group after 3 weeks of intervention(P<0.05).However,HR and RPE scores were lower in the observation group than the control group(P<0.05).The patient satisfaction rates on rehabilitation treatment were higher in the observation group than those in the control group(P<0.05).Conclusion:Exoskeleton robot-assisted rehabilitation combined with conventional physical therapy can significantly improve lower limb rehabilitation outcomes in stroke patients.It can enhance lower limb motor function,balance ability,activities of daily living,and walking ability with high patient satisfaction.
