巨噬细胞极化机制对特发性肺纤维化的影响

摘要 特发性肺纤维化(IPF)是一种以进行性肺组织纤维化为特征的致死性疾病,其发病机制复杂且目前缺乏有效治疗手段。巨噬细胞在IPF的发生与发展中发挥关键作用,尤其是其极化状态的变化。经典活化的促炎巨噬细胞在IPF早期发挥促炎和抗感染作用,而选择性活化的抗炎巨噬细胞在IPF进展过程中通过促进成纤维细胞活化与胶原合成,加剧肺纤维化。本文综述了巨噬细胞极化的分子机制、信号通路及其在IPF中的功能角色,并探讨了调控巨噬细胞极化作为潜在治疗策略的研究进展与前景,以为IPF的靶向治疗提供理论依据。 Idiopathic pulmonary fibrosis(IPF)is a fatal disease characterized by progressive fibrosis of lung tissue.Its pathogenesis is complex and there is currently a lack of effective treatment methods.Macrophage play a key role in the occurrence and development of IPF,especially in the changes of their polarization state.Classically activated pro-inflammatory macrophage exert pro-inflammatory and anti-infective effects in the early stage of IPF,while selectively activated anti-inflammatory macrophage exacerbate pulmonary fibrosis during the progression of IPF by promoting fibroblast activation and collagen synthesis.This article reviews the molecular mechanism,signaling pathways of macrophage polarization and its functional role in IPF,and explores the research progress and prospect of regulating macrophage polarization as a potential therapeutic strategy,providing a theoretical basis for targeted therapy of IPF.
