“双碳”背景下县级公立医院“费用托管-结构重塑”双轮驱动模式研究——以溧阳市人民医院为例

摘要 论文提出“费用托管—结构重塑”双轮驱动模式:通过费用托管机制解决资金与管理短板,以能源系统结构优化实现多能互补与智能化调控,构建“外循环—内循环”的闭环路径。以溧阳市人民医院为例,开展了系统能源审计与节能改造实践,分析了生活热水、蒸汽及中央空调系统的结构性问题,并实施空气源热泵替代、蒸汽直供、冷热源分区及群控系统等优化措施。结果表明,改造后医院综合能耗同比下降18.65%,能源费用减少179.06万元,显著提升了能效水平与运行可靠性。研究揭示了托管机制与结构优化协同作用的可行性与成效,建立了制度保障与技术优化耦合的运行机理。为县级公立医院能源管理与低碳转型提供了实践路径,也为公共机构节能降碳模式提供了参考。 The paper proposes a dual-driven model of“Cost Trusteeship–Structural Remodeling.”Through the trusteeship mechanism,financial and managerial constraints are addressed,while optimization of the energy system structure enables multi-energy complementarity and intelligent regulation,forming a closed-loop pathway of“external circulation–internal circulation.”Taking Liyang People’s Hospital as a case study,a comprehensive energy audit and retrofitting practice was conducted.The analysis focused on structural problems in the domestic hot water,steam,and central air-conditioning systems,and optimization measures were implemented,including air-source heat pump replacement,direct steam supply,zoning of cooling and heating sources,and the adoption of a group control system.The results show that,after the retrofit,the hospital’s total energy consumption decreased by 18.65%year-on-year,energy costs were reduced by 1.79 million CNY,and energy expenditure per 10,000 CNY of income decreased by more than 20%,significantly improving energy efficiency and operational reliability.The study demonstrates the feasibility and effectiveness of integrating trusteeship mechanisms with structural optimization,establishing an operation mechanism that couples institutional guarantees with technological enhancement.This research provides a practical pathway for energy management and low-carbon transformation in county-level public hospitals and offers a reference model for energy conservation and carbon reduction in public institutions.
