摘要
目的:探讨影响早产儿万古霉素血药谷浓度的因素。方法:选取2021年1月—2025年3月于徐州市妇幼保健院新生儿科接受万古霉素血药浓度监测的早产儿82例,按标准筛选后,根据10~15 mg/kg体质量给药,于第5剂给药前30 min采血,采用化学发光免疫法检测万古霉素血药谷浓度,进行单因素及多因素Logistic回归分析。结果:单因素分析显示,谷浓度较高组和谷浓度较低组胎龄、体质量、血清肌酐、用药时间比较,差异有统计学意义(P<0.05);多因素Logistic回归分析证实,胎龄28~30周、体质量1000~1300 g、血清肌酐>45μmol/L、用药时间>10 d是影响早产儿万古霉素血药谷浓度的独立影响因素(P<0.05)。结论:胎龄、出生体质量、肾功能及用药时间显著影响早产儿万古霉素血药谷浓度。临床用药应结合血药浓度监测,制定个体化方案以指导临床安全用药。
Objective:To explore the influencing factors of vancomycin trough concentration in premature infants.Methods:A total of 82 premature infants who were monitored for the plasma drug concentration of vancomycin in Department of Neonatology at Xuzhou Maternal and Child Health Hospital from January 2021 to March 2025 were selected.After standard screening,the infants were administered a dose of 10-15 mg/kg body weight,and blood samples were collected 30 min before the 5th dose.The trough concentration of vancomycin was measured by chemiluminescence immunoassay,and univariate and multivariate Logistic regression analyses were performed.Results:The univariate analysis showed statistically significant differences in gestational age,body weight,serum creatinine and duration of medication between the high trough concentration group and the low trough concentration group(P<0.05).Multivariate Logistic regression analysis confirmed that,gestational age of 28-30 weeks,body weight of 1000-1300 g,serum creatinine>45μmol/L,and duration of medication>10 d were independent influencing factors of vancomycin trough concentration in premature infants(P<0.05).Conclusion:Gestational age,birth weight,renal function,and duration of medication significantly affect the trough concentration of vancomycin in premature infants.Clinical medication should be combined with plasma concentration monitoring to develop individualized treatment plans,thus guiding safe clinical medication.
