摘要
目的:分析腹腔镜下保留左结肠动脉(LCA)直肠癌根治术治疗中低位原发性直肠癌的效果。方法:采取随机数字表法将2022年1月—2024年5月于丰县人民医院进行手术的78例中低位原发性直肠癌患者分为对照组(39例)和观察组(39例)。对照组行腹腔镜下不保留LCA直肠癌根治术,观察组行腹腔镜下保留LCA直肠癌根治术。对比两组临床指标、并发症发生情况、炎性因子水平及治疗效果。结果:观察组术中出血量少于对照组(P<0.001),操作时间、疼痛消失时间、术后下床活动时间及住院时间短于对照组(P<0.001);观察组并发症发生率较对照组低(P=0.033);术后3 d,观察组白细胞介素-10、C反应蛋白、白细胞计数水平低于对照组(P<0.001);观察组治疗总有效率高于对照组(P=0.033)。结论:腹腔镜下保留LCA直肠癌根治术治疗中低位原发性直肠癌效果显著,能够减少术中出血量,缩短手术时间和术后恢复时间,减轻炎性反应,且安全性良好。
Objective:To analyze the effect of laparoscopic rectal cancer radical resection preserving left colic artery(LCA)in treatment of middle and low primary rectal cancer.Methods:According to random number table method,a total of 78 patients with middle and low primary rectal cancer who underwent surgery in Fengxian People's Hospital from January 2022 to May 2024 were divided into control group(39 cases)and observation group(39 cases).The control group underwent laparoscopic rectal cancer radical resection without the LCA preservation,and the observation group underwent laparoscopic rectal cancer radical resection with the LCA preservation.The clinical indexes,occurrence of complications,levels of inflammatory factors and therapeutic effect of the two groups were compared.Results:Compared with the control group,the intraoperative blood loss in the observation group was less(P<0.001),and the operation time,pain relief time,postoperative ambulation time,and hospital stay were shorter(P<0.001).Compared with the control group,the incidence of complications in the observation group was lower(P=0.033).Compared with the control group,the levels of interleukin-10,C-reactive protein,and white blood cell count on the 3rd day after surgery in the observation group were lower(P<0.001).Compared with the control group,the total effective rate of treatment of the observation group was higher(P=0.033).Conclusion:Laparoscopic rectal cancer radical resection preserving LCA shows significant effects in the treatment of middle and low primary rectal cancer,and can reduce intraoperative blood loss,shorten operation time and postoperative recovery time,alleviate inflammatory response,and maintain good safety.
