摘要
目的分析不同种类GnRHa治疗特发性中枢性性早熟(ICPP)女童的效果,分析其对患儿生长指标及子宫体积的影响。方法选取2022年1月-2024年6月信丰县人民医院收治的40例ICPP女童为研究对象,采用随机数字表法分为对照组与研究组,每组20例。对照组予以皮下注射亮丙瑞林注射液治疗,研究组予以肌肉注射曲普瑞林注射液治疗。比较两组生长指标[骨龄进展、身高标准差评分(Ht-SD)、体重、BMI]、子宫及卵巢体积变化以及不良反应(头晕、胃肠道反应、盗汗)发生率。结果治疗后,两组骨龄进展速率均较治疗前减慢(P<0.05),且研究组骨龄进展速率低于对照组(P<0.05);两组Ht-SD均较治疗前升高(P<0.05),研究组Ht-SD高于对照组(P<0.05);两组体重、BMI比较,差异无统计学意义(P>0.05)。治疗后,两组子宫体积、卵巢体积均较治疗前缩小(P<0.05),研究组子宫体积、卵巢体积均小于对照组(P<0.05)。研究组不良反应发生率低于对照组(P<0.05)。结论亮丙瑞林与曲普瑞林治疗ICPP女童均能有效抑制性发育、改善生长潜力,但曲普瑞林在减缓骨龄进展、提升身高SD值及缩小生殖器官体积方面疗效更优,且不良反应发生率更低,安全性更高,可作为ICPP女童临床治疗的优选方案之一。
Objective To analyze the effects of different types of GnRHa on the growth index and uterine volume of girls with idiopathic central precocious puberty(ICPP).Methods A total of 40 ICPP girls admitted to Xinfeng County People′s Hospital from January 2022 to June 2024 were selected as the research objects.They were divided into control group and study group by random number table method,with 20 girls in each group.The control group was treated with subcutaneous injection of leuprorelin injection,and the study group was treated with intramuscular injection of triptorelin injection.The growth indexes[bone age progression,height standard deviation score(Ht-SD),body weight,BMI],uterine and ovarian volume changes and the incidence of adverse reactions(dizziness,gastrointestinal reactions,night sweats)were compared between the two groups.Results After treatment,the rate of bone age progression in both groups was slower than that before treatment(P<0.05),and the rate of bone age progression in the study group was lower than that in the control group(P<0.05).The Ht-SD of the two groups was higher than that before treatment(P<0.05),and the Ht-SD of the study group was higher than that of the control group(P<0.05).There was no significant difference in body weight and BMI between the two groups(P>0.05).After treatment,the uterine volume and ovarian volume of the two groups were smaller than those before treatment(P<0.05),and the uterine volume and ovarian volume of the study group were smaller than those of the control group(P<0.05).The incidence of adverse reactions in the study group was lower than that in the control group(P<0.05).Conclusion Leuprorelin and triptorelin can effectively inhibit sexual development and improve growth potential in the treatment of ICPP girls,but triptorelin is more effective in slowing down the progression of bone age,increasing the height SD value and reducing the volume of reproductive organs,and the incidence of adverse reactions is lower and the safety is higher.It can be used as one of the preferred schemes for clinical treatment of ICPP girls.
