摘要
目的:探讨人工腱索技术在机器人辅助二尖瓣成形术中的安全性及有效性,并总结该术式的初步经验。方法:回顾性分析2016年12月—2024年1月于南京医科大学第一附属医院因二尖瓣关闭不全行达芬奇机器人辅助二尖瓣成形术的70例患者临床资料。根据有无使用人工腱索,将患者分为人工腱索组(n=54)和非人工腱索组(n=16),比较两组患者的围手术期相关指标及随访结果。结果:患者均成功完成手术。人工腱索组平均植入1.83根腱索,阻断时间长于非人工腱索组(P<0.05);人工腱索组有3例(5.56%)患者合并行三尖瓣成形术,该比例低于非人工腱索组的4例(25.00%),差异具有统计学意义(P<0.05)。随访12个月及18个月,两组患者二尖瓣反流情况比较,差异无统计学意义(P>0.05);两组术后并发症发生情况比较,差异无统计学意义(P>0.05)。结论:机器人辅助二尖瓣成形术中植入人工腱索是安全有效的,与非人工腱索技术相似,均可获得良好的随访结果。
Objective:To explore the safety and efficacy of artificial chordae in robot-assisted mitral valvuloplasty,and to summarize the preliminary experience with this surgical procedure.Methods:A retrospective analysis was conducted on clinical data from 70 patients who underwent Da Vinci robot-assisted mitral valvuloplasty for mitral regurgitation at the First Affiliated Hospital with Nanjing Medical University from December 2016 to January 2024.Patients were divided into the artificial chordae group(n=54)and the non-artificial chordae group(n=16).Perioperative indicators and follow-up results were compared between the two groups.Results:All surgeries were successfully completed.The artificial chordae group had an average of 1.83 chordae implanted,with a longer cross-clamp time than the non-artificial chordae group(P<0.05).Three cases(5.56%)in the artificial chordae group underwent concurrent tricuspid valvuloplasty,a proportion significantly lower than that of the non-artificial chordae group(4 cases,25.00%)(P<0.05).Follow-up at 12 and 18 months showed no statistically significant difference in mitral regurgitation between the two groups(P>0.05).There was no significant difference in postoperative complications between the two groups(P>0.05).Conclusion:The application of artificial chordae in robot-assisted mitral valvuloplasty is safe and effective,and it can achieve favorable follow-up results comparable to non-artificial chordae techniques.
