摘要
目的探究沙库巴曲缬沙坦对老年高血压肾病患者血肌酐(SCr)、白细胞介素6(IL-6)的影响。方法纳入2019年10月至2024年8月湖北省荣军医院收治的100例高血压肾病老年患者,随机分为对照组(50例)和观察组(50例)。两组均进行生活管理,对照组口服缬沙坦片治疗,观察组口服沙库巴曲缬沙坦片治疗。比较两组疗效、血压(收缩压和舒张压)、肾功能[24 h尿蛋白、血肌酐(SCr)和胱抑素C(Cys-C)]、炎症因子[白细胞介素6(IL-6)、IL-1β、肿瘤坏死因子α(TNF-α)]及不良反应情况。结果观察组治疗总有效率为90.00%,高于对照组的74.00%,差异有统计学意义(P<0.05)。治疗后观察组收缩压、舒张压、24 h尿蛋白、SCr、Cys-C、IL-6、IL-1β和TNF-α低于对照组,差异有统计学意义(P<0.05)。两组不良反应总发生率比较差异无统计学意义(P>0.05)。结论沙库巴曲缬沙坦治疗老年高血压肾病疗效优于缬沙坦,在降压、改善肾功能和减轻炎症反应方面效果更佳,且安全性良好。
Objective To explore the effects of sacubitril valsartan on serum creatinine(SCr)and interleukin-6(IL-6)in elderly patients with hypertensive nephropathy.Methods A total of 100 elderly patients with hypertensive nephropathy were admitted to Hubei Provincial Rongjun Hospital and randomly divided into a control group(50 cases)and an observation group(50 cases)between October 2019 and August 2024.Based on lifestyle management,the control group received oral valsartan tablets,while the observation group received oral sacubitril valsartan tablets.The curative effect,blood pressure(systolic and diastolic),renal function(24h urinary protein,SCr,cystatin C(Cys-C),inflammatory factors(IL-6,IL-1β,tumor necrosis factorα(TNF-α),and adverse reactions were compared between the two groups.Results The total response rate to treatment was higher in the observation group than in the control group(90.00%vs 74.00%,P<0.05).After treatment,sys-tolic blood pressure,diastolic blood pressure,24 h urinary protein,SCr,Cys-C,IL-6,IL-1β,and TNF-αwere lower in the observation group compared to the control group(P<0.05).The difference in total incidence of adverse reactions between the two groups was not statistically significant(P>0.05).Conclusion The cura-tive effect of sacubitril valsartan is better than that of valsartan in elderly patients with hypertensive nephropathy.It has superior effects in lowering blood pressure,improving renal function,and relieving inflammatory re-sponse,while also maintaining good safety.
