摘要
目的 了解2010—2023年珠海市生殖道沙眼衣原体感染的流行趋势及空间分布特征,旨在为该疾病的精准防控提供科学策略。方法 依托中国疾病预防控制信息系统,收集2010—2023年珠海市生殖道沙眼衣原体感染病例数据,应用描述性流行病学法分析流行特征,运用χ^(2)_(趋势)检验进行趋势分析,检验水准α=0.05,并采用空间自相关分析法探索病例空间聚集模式。结果 2010—2023年珠海市累计报告23 815例生殖道沙眼衣原体感染病例,报告发病率先上升(从2010年的56.84/10万增至2019年的121.99/10万),后下降至2023年的81.47/10万;女性病例占比62.28%,男女性别比由2010年的1:1.01增至2023年的1:1.80;发病年龄以20~39岁为主,占比82.91%;家务及待业、商业服务和工人为主要感染群体,合计占比70.34%;病例主要分布于拱北街道、吉大街道、翠香街道、前山街道,合计占比55.27%;空间自相关分析显示,感染病例存在显著的空间聚集性,全局Moran’s I值为0.176(Z=3.411,P<0.001),具体表现为“高-高”聚集区(拱北、吉大街道)与“低-高”聚集区(湾仔、梅华、狮山、香湾街道)并存。结论 2010—2023年珠海市生殖道沙眼衣原体感染报告发病率呈先上升后下降趋势,女性、20~39岁、家务及待业人员为感染多发群体,病例空间聚集性明显。
Objective To understand the prevalence trend and spatial distribution characteristics of genital Chlamydia trachomatis infection in Zhuhai City from 2010 to 2023,and to provide scientific basis for the precise prevention and control of the disease.Methods Case data of genital Chlamydia trachomatis infection in Zhuhai City from 2010 to 2023 were collected based on the China Disease Prevention and Control Information System.Epidemiological characteristics were analyzed by descriptive epidemiology.Trend analysis was carried out byχ^(2) trend test,and the test level wasα=0.05.Spatial autocorrelation analysis was used to study the spatial aggregation of cases.Results From 2010 to 2023,the cumulative number of reported cases of genital Chlamydia trachomatis infection in Zhuhai City was 23815,and the reported incidence increased first(from 56.84/105 in 2010 to 121.99/105 in 2019),and then decreased to 81.47/105 in 2023.Female cases accounted for 62.28%,and the sex ratio between men and women increased from 1:1.01 in 2010 to 1:1.80 in 2023.The age of onset was mainly 20-39 years old,accounting for 82.91%.Household and unemployed,business services and workers were the main infected groups,accounting for 70.34%.Cases were mainly distributed in Gongbei Street,Jida Street,Cuixiang Street and Qianshan Street,accounting for 55.27%.Spatial autocorrelation analysis showed that there was a significant spatial aggregation of infected cases,and the global Moran's I value was 0.176(Z=3.411,P<0.001),which specifically characterized by the coexistence of“high-high”aggregation areas(Gongbei Street and Jida Street)and“low-high”aggregation areas(Wanzai,Meihua,Shishan and Xiangwan Streets).Conclusions From 2010 to 2023,the reported incidence of genital Chlamydia trachomatis infection in Zhuhai City increased first and then decreased.Females,persons aged 20-39 years and household and unemployed workers were the most infected groups,and the spatial aggregation of cases was obvious.
