摘要
目的分析厦门市医疗机构住院患者静脉输液使用现状,为规范静脉输液管理提供依据。方法收集2023年全市20家二级及以上医疗机构住院患者静脉输液使用监测数据,采用回顾性研究方法,分析不同性质(公立vs.民营)或者不同类别(综合vs.专科)医疗机构住院患者静脉输液使用现状。结果纳入统计的20家医疗机构住院患者静脉输液使用率中位值为78.17%、非手术住院患者静脉输液使用率中位值为71.31%、静脉输液床日占比中位值为69.83%、静脉输液平均每床日使用数量中位值为2.01袋/瓶、静脉输液平均每床日使用体积中位值为407.06 mL。中药注射剂静脉输液使用率中位值为0.02%、质子泵抑制药注射剂静脉输液使用率中位值为10.72%、肠外营养静脉输液使用率中位值为5.94%、单瓶输注肠外营养使用占比中位值为83.40%、使用止吐药物静脉输液使用率中位值为2.33%。结论住院患者静脉输液使用率、平均每床日使用数量等指标总体低于2020年国内一项调研的结果,但是仍存在肠外营养单瓶输注营养比例过高、营养输注方式欠规范等问题,建议参照国内外指南推广全合一营养输注方式,同时,建议各地行业学会、质控中心出台指南共识及评价标准加强静脉输液规范使用的指导。
OBJECTIVE To analyze the cu rrent status of intravenous infusion utilization in hospitalized patients in Xiamen medical institutions and pr ovide evidence for standardizing intr avenous infusion management.METHODS Monitoring data on intravenous infusi on utilization in hospitalized patien ts from 20 secondary and above medical institutions in 2023 were collected.A retro spective study was conducted to analy ze the current status of intravenous i nfusion utilization in hospitals of diff erent natures(public vs.private)or ca tegories(general vs.specialized).RESU LTS Among the 20 institutions,the median values of key indicators were as follo ws:intravenous infusion utilization r ate in hospitalized patients(78.17%),non-sur gical hospitalized patients(71.31%),b ed-day proportion of intravenous infusion(69.83%),average daily volume p er bed(2.01 bags/bottles),and average daily volume per bed(407.06 mL).For specific medications,the medi an utilization rates were:traditional Chinese medicine injections(0.02%),proton pump inhibitor injections(10.72%),parenteral nutrition(5.94%),singl e-bottle parenteral nutrition infusion(83.40%),and antiemetic drug-asso ciated intravenous infusions(2.33%).C ONCLUSION The intravenous infusion utilization rate and daily u sage volume per bed in hospitalized pa tients were generally lower than thos e reported in a 2020 domestic study.However,i ssues such as excessive single-bottle parenteral nutrition infusion and non standardized nutrition administratio n methods persist.It is recommended to promote the all-in-one nutrition infus ion method in accordance with domestic an d international guidelines.Additiona lly,industry associations and qualit y control centers should establish guideli nes,consensus,and evaluation standar ds to strengthen the standardized use of intravenous infusions.
