摘要
目的分析临床药师参与抗感染会诊的特点、趋势及效果,为临床药师会诊提供借鉴,改善会诊质量提供参考。方法根据会诊申请单,通过病例系统收集2024年1-6月临床药师会诊患者资料,统计分析患者的一般资料、感染部位、病原菌检出、会诊科室、会诊目的、临床药师会诊意见、意见采纳情况以及转归情况。结果参与会诊的患者多为老年患者(≥60岁),男性居多。13个科室提出临床药师会诊申请,会诊患者以皮肤软组织感染、骨关节感染及呼吸系统感染为主。金黄色葡萄球菌为主要致病菌,其次为铜绿假单胞菌。会诊目的主要为细菌培养阳性选定抗菌药物。药师会诊意见主要为调整抗菌药物类型。药师会诊意见采纳率为93.8%,有效率为93.4%,与未采纳有效率之间存在显著差异(P<0.05)。结论临床药师参与抗感染会诊已被临床医师普遍接受,临床药师能够应用专业药学知识,为患者提供个体化药学服务,促进抗菌药物合理应用,对改善患者预后具有重要意义。
Objective To analyze the characteristics,practices and effects of pharmaceutical consultations involving clinical pharmacists,sum up the experience and provide a reference for improving the quality of pharmaceutical consultations.Methods According to consultation applications,the data on consultations attended by clinical pharmacists between January and June in 2024 was retrospectively collected and analyzed through the case system,including the basic information of patients,infection sites,detection of pathogenic bacteria,departments involved in consultations,purposes of consultations,pharmacists′opinions,compliance and outcomes.ResultsMost of the patients involved in consultations were elderly(≥60 years old)and male.Thirteen departments applied for pharmaceutical consultations,mostly because of infections related to the skin and soft tissues,bone and joints and the respiratory system.Staphylococcus aureus was the dominating pathogenic bacteria,followed by Pseudomonas aeruginosa.The chief objective of clinical consultations was to select antibiotics with positive bacterial culture.The pharmacists′tips revolved around adjustment of the types of antibiotics.The compliance rate was 93.8%,and the effective rate was 93.4%,so the difference was statistically significant(P<0.05).Conclusion The participation of clinical pharmacists in anti-infection consultations has been widely accepted by clinicians,and is of great significance to improve the prognosis of patients by using their pharmaceutical knowledge,providing individualized pharmaceutical care and promoting the rational use of antibiotics.
