摘要
慢性肾脏疾病(chronic Kidney Disease,CKD)是由多种原因引起的肾组织损伤,进而引发肾单位破坏及肾功能下降,最终发展为肾衰竭。CKD的发病机制不完全清楚,且现代医学在临床治疗学上仍缺乏理想治疗药物。因此预防和延缓CKD的发生发展是当前亟待解决的重大问题。AMP活化蛋白激酶(AMPK)的活性对于维持肾细胞(包括足细胞和肾小管细胞)的生理功能至关重要,可从多种途径干预CKD的进展,其中细胞自噬是其干预CKD的重要途径之一。因此,研究探讨AMPK通路介导的细胞自噬在CKD中的作用,对于疾病的治疗有着重要意义。中药在我国CKD临床治疗实践中起重要的作用,其疗效不断受到临床实践和研究的肯定,受到研究者的广泛关注,且中药基于该机制防治CKD的研究已取得较大进展。因此笔者从AMPK通路介导细胞自噬途径出发,阐明其干预CKD的关键机制及相关研究进展,并整理归纳了基于该机制的中药复方、中药单体活性成分或提取物治疗CKD的方法,挖掘其潜在的干预机制及治疗意义,以期为中医药防治CKD、后续的实验研究及新药研发提供更多的理论依据与新思路。
Chronic kidney disease(CKD)is kidney tissue damage caused by various factors,leading to nephron destruction and a decline in renal function,ultimately resulting in renal failure.The pathogenesis of CKD is not completely understood,and modern medicine still lacks ideal therapeutic drugs for clinical treatment.Therefore,preventing and delaying the occurrence and progression of CKD is a significant issue that urgently needs to be addressed.The activity of AMP-activated protein kinase(AMPK)is crucial for maintaining the physiological function of renal cells,including podocytes and renal tubular cells,and it can intervene in the progression of CKD through various pathways,with autophagy being one of the important mechanisms.Thus,studying the role of AMPK pathway-mediated autophagy in CKD is of great significance for disease treatment.Traditional Chinese medicine(TCM)plays an important role in the clinical treatment of CKD in China.Its efficacy has been continuously affirmed by clinical practice and research,garnering widespread attention from researchers.Moreover,significant progress has been made in the research on the prevention and treatment of CKD using TCM based on this mechanism.This paper clarifies the key mechanisms of AMPK pathway-mediated autophagy in the intervention of CKD,summarizes relevant research progress,organizes and categorizes the methods of treating CKD with TCM compounds,active components,or extracts of TCM monomers based on this mechanism,exploring their potential intervention mechanisms and therapeutic significance.The study is expected to provide a theoretical basis and new ideas for the prevention and treatment of CKD using TCM,as well as for subsequent experimental research and new drug development.
