摘要
早发性卵巢功能不全(POI)是指女性40岁前出现生殖功能下降,由于女性社会及生活压力增大,加之久坐、熬夜等不良生活习惯,发病率呈逐年上升趋势。中医学认为POI的主要病机是冲任虚损、督脉阳衰,或肝郁脾虚、肾气亏损。针刺治疗POI以补肾调冲任为大法,重在调治脏腑功能,恢复卵巢功能。冲任虚损、督脉阳衰者调理冲、任、督三脉,采用“调任通督针刺法”“补肾调理冲任针刺法”等针刺方法;肝郁脾虚、肾气亏损者疏肝理脾,补肾益精,采用“调经促孕针刺法”,临床疗效显著;肾阴肾阳亏虚者滋肾补肾,活血化瘀,采用温针灸。文章整理归纳近年相关研究,发现针刺可以调节下丘脑-垂体-卵巢轴的内分泌功能,增加卵巢储备,提高子宫内膜容受性,促进卵泡发育,改善辅助生殖妊娠结局。文章通过综述针刺在POI治疗方面的研究,分析其作用机制,旨在为针刺治疗POI提供更多理论依据。
Premature ovarian insufficiency(POl)refers to a decline in reproductive function in females before the age of 40.Due to increased social and life pressures on women,coupled with sedentary lifestyles,staying up late,and other unhealthy habits,the incidence of POI is steadily increasing.Traditional Chinese medicine(TCM)considers the main pathogenesis of POI to be deficiency in the Chong and Ren meridians,along with weakness in the Du meridian,or liver depression and spleen deficiency and kidney qi deficiency.Acupuncture treatment for POI primarily focuses on tonifying the kidney and regulating the Chong and Ren meridians to restore ovarian function.For those with deficiency in the Chong and Ren meridians or weakness in the Du meridian,acupuncture methods such as'regulating Ren and stimulating Du needling technique'and'kidney nourishment and Chong-Ren regulation needling technique'are employed.For those with liver depression and spleen deficiency or kidney qi deficiency,acupuncture aims to soothe the liver,regulate the spleen,tonify the kidneys,and replenish essence using techniques like'regulating menstruation and promoting conception needling technique',which has shown significant clinical efficacy.For those with deficiency of kidney yin and yang in TCM,tonify the kidneys,promote blood circulation,resolve stasis,utilizing warming needle moxibustion.This paper summarizes recent relevant research,revealing that acupuncture can regulate the hypothalamicpituitary-ovarian axis,increase ovarian reserves,enhance endometrial receptivity,promote follicle development,and improve outcomes in assisted reproductive technology.By reviewing acupuncture's role in treating POI and analyzing its mechanisms of action,this paper aims to provide a stronger theoretical basis for acupuncture treatment of POI.
