摘要
文章旨在总结房繄恭教授运用调经促孕针法治疗卵巢储备功能减退(DOR)不孕的临床经验。房繄恭教授认为,DOR不孕主要病机为冲任失调,责之肾虚、肝郁、脾虚。所创调经促孕针法立足冲任,强调补肝肾、安神志,取穴中脘、关元、大赫调补冲任,配伍天枢、足三里调理脾胃,以助气血生化有源,肾俞、中髎、大赫、太溪培补肾气,三阴交通调肝脾肾,太冲疏肝解郁,百会、神庭、本神安神定志,加之不孕效穴阴廉、子宫,诸穴合用,共奏调经助孕之功。同时,针刺过程中重视运用逍遥调神手法,强调医患双方治神守神,重视全周期治疗。
This article aims to summarize the clinical experience of professor FANG Yigong in treating infertility caused by diminished ovarian reserve(DOR)with the Tiaojing Cuyun acupuncture technique.Professor FANG Yigong holds that the core pathogenesis of DOR-related infertility lies in the disharmony of Chong and Ren Meridians,which is mainly attributed to kidney deficiency,liver stagnation,and spleen deficiency.The self-developed Tiaojing Cuyun acupuncture technique is based on regulating Chong and Ren Meridians,with emphasis on nourishing the liver and kidney and calming the mind.Specific acupoints are selected as follows:Zhongwan(CV12),Guanyuan(CV4),and Dahe(KI12)are used to regulate and tonify Chong and Ren Meridians;Tianshu(ST25)and Zusanli(ST36)are combined to regulate the spleen and stomach,ensuring sufficient production of qi and blood;Shenshu(BL23),Zhongliao(BL33),Dahe(KI12),and Taixi(KI3)are applied to tonify kidney qi;Sanyinjiao(SP6)is used to harmonize the liver,spleen,and kidney;Taichong(LR3)is selected to soothe the liver and relieve stagnation;Baihui(GV20),Shenting(GV24),and Benshen(GB13)are adopted to calm the mind and stabilize the will.In addition,Yinlian(LR11)and Zigong(EX-CA1),the effective acupoints for infertility,are included.The combined use of these acupoints achieves the therapeutic effect of regulating menstruation and promoting fertility.Meanwhile,during acupuncture,the Xiaoyao Tiaoshen needling technique is emphasized,along with the importance of both the doctor and the patient focusing on'regulating the mind and preserving the spirit'.The whole-cycle treatment is also valued in the clinical practice.
