摘要
目的:运用数据挖掘技术分析近10年中医药从脾论治2型糖尿病(type 2 diabetes mellitus,T2DM)的组方特点与用药规律,探讨其临床应用价值。方法:检索中国期刊全文数据库(CNKI)、万方数据库、维普中文科技期刊数据库2015年1月1日—2025年1月1日期间公开发表的、基于脾胃理论治疗T2DM的临床研究及医案文献,筛选符合纳入标准的方剂并建立数据库。利用古今医案云平台V2.3.9对处方中的高频用药、性味归经分布、药对关联及复杂网络结构进行分析。结果:共纳入330篇文献,提取处方330首,涉及中药265味。高频方剂有升阳益胃汤(15%)、六君子汤(11%)、参苓白术散(8%);高频药物为白术(71.52%)、茯苓(70.61%)、黄芪(59.39%);药性以平、温、微寒为主;药味多为甘、苦、辛;归经以脾、肺、胃、肝为主。关联规则分析显示,白术-茯苓、白术-黄芪、黄芪-茯苓等为核心药对;复杂网络分析揭示,核心药物网络以健脾益气、化湿理气为中心,兼顾清热、活血、滋阴等法。结论:纳入的方剂规律体现出以健脾调中为本、标本兼顾、攻补兼施的治疗思路,印证了“脾为后天之本、气血生化之源”的理论指导意义,为现代T2DM的辨证施治与方药优化提供了数据支持与参考依据,并为后续临床研究与大数据分析奠定了基础。
Objective To analyze the formula characteristics and medication patterns of TCM treatment for type 2 diabetes mellitus(T2DM)based on the spleen theory in the past decade through data mining technology,and to explore its clinical application value.Methods Clinical research and medical case literature on the treatment of T2DM based on the spleen and stomach theory published from January 1,2015 to January 1,2025 in the China National Knowledge Infrastructure(CNKI),Wanfang Database,and VIP Chinese Science and Technology Journal Database were retrieved.Formulas meeting the inclusion criteria were screened and a database was established.The Ancient and Modern Medical Case Cloud Platform V2.3.9 was used to analyze the high-frequency drugs,the distribution of properties and meridians,drug pair associations,and complex network structures in the prescriptions.Results A total of 330 articles were included,and 330 prescriptions were extracted,involving 265 kinds of Chinese medicines.The high-frequency formulas were Shengyang Yiwei Decoction(15%),Liujunzi Decoction(11%),and Shenling Baizhu Powder(8%);the high-frequency drugs were Atractylodes macrocephala(71.52%),Poria cocos(70.61%),and Astragalus membranaceus(59.39%);the properties were mainly neutral,warm,and slightly cold;the flavors were mostly sweet,bitter,and pungent;the meridians mainly involved the spleen,lung,stomach,and liver.Association rule analysis showed that Atractylodes macrocephala-Poria cocos,Atractylodes macrocephala-Astragalus membranaceus,and Astragalus membranaceus-Poria cocos were the core drug pairs;complex network analysis revealed that the core drug network centered on strengthening the spleen and benefiting Qi,regulating Qi and removing dampness,and also included methods such as clearing heat,promoting blood circulation,and nourishing Yin.Conclusion The treatment ideas reflected in the formulas included in this study are centered on strengthening the spleen and regulating the middle,taking both the root and the branch into account,and combining attack and tonification.This confirms the guiding significance of the theory that"the spleen is the root of the body and the source of Qi and blood production",provides data support and reference basis for the modern TCM syndrome differentiation and treatment of T2DM and the optimization of formulas and drugs,and also lays the foundation for subsequent clinical research and big data analysis.
