摘要
皮肤病尤其是慢性难治性皮肤病,多因脏腑功能失调,经络阻滞、气血失和,肌肤腠理失疏,内生风湿热瘀毒而发病。作为八法之一的“和法”,可调畅气机、平衡阴阳、调和脏腑功能。“调和”是中医外治法治疗皮肤病的关键。其具体应用表现为:针对阴阳失调、营卫失和之皮肤病患者,以和营卫、调表里、消斑疹为治法,常施以穴位注射、自血疗法处之。针对气血失和、经络阻滞之皮肤病患者,以和气血、调经络、化瘀毒为治法,常予拔罐走罐、刺络放血处之。针对肝脾失调、气机不畅、风湿热蕴阻之皮肤病患者,以和肝脾、调气机、祛风湿为治法,常予溻渍、针灸疗法处之。针对上热下寒证之皮肤病患者,以和上下、调寒热、治痤痱为治法,常予耳尖放血结合中药面膜、脐周艾灸处之。本文结合临床验案对从“和法”角度应用外治法治疗皮肤病进行阐述。
Dermatological diseases,especially chronic and refractory skin conditions,often arise due to visceral dysfunction,meridian obstruction,imbalance of qi and blood,disruption of skin and interstices,and internal generation of wind,dampness,heat,stasis,and toxins.As one of the eight therapeutic method in Traditional Chinese Medicine(TCM),the harmonizing method regulates qi movement,balances yin and yang,and harmonizes visceral functions.Harmonization is the key principle of TCM external treatments for dermatological diseases.Its specific applications include:For patients with imbalance of yin and yang and disharmony between the nutritive and defensive levels,treatments such as acupoint injection and autohemotherapy are often used to harmonize the nutritive and defensive levels,regulate the exterior and interior,and resolve macules and rashes.For patients with imbalance of qi and blood and meridian obstruction,cupping,moving cupping,and pricking bloodletting are commonly applied to harmonize qi and blood,regulate meridians,and resolve stasis and toxins.For patients with liver-spleen dysfunction,disrupted qi movement,and internal obstruction of wind,dampness,and heat,treatments such as herbal soaking and acupuncture are often employed to harmonize the liver and spleen,regulate qi movement,and dispel wind and dampness.For patients with the upper heat and lower cold pattern,ear tip bloodletting combined with TCM herbal masks and periumbilical moxibustion are commonly used to harmonize the upper and lower body,regulate cold and heat,and treat acne and miliaria.Clinical case studies are also included to illustrate the application of external treatments from the perspective of the harmonizing method in dermatological diseases.
