摘要
目的:探讨基于关爱和支持的叙事护理对继发性肺结核患者心理弹性与自我管理的改善效果.方法:选取2023年1月至2025年1月本院收治的68例继发性肺结核患者作为研究对象.按护理方法,将其分为研究组(n=34,接受基于关爱和支持的叙事护理)和对照组(n=34,接受常规护理).比较两组患者心理弹性[采用中文版Connor-Davidson心理弹性量表(CD-RISC)评估]、自我管理[采用慢性病自我管理量表(CDSMS)评估]及应对方式[采用医学应对问卷(MCMQ)评估].结果:护理后,两组患者CD-RISC、CDSMS各维度评分均升高,研究组上述维度评分高于对照组,差异有统计学意义(P<0.05).与对照组比较,研究组患者护理后MCMQ中面对维度评分更高,回避、屈服维度评分更低,差异有统计学意义(P<0.05).结论:基于关爱和支持的叙事护理可显著提升继发性肺结核患者的心理弹性,增强自我管理能力,并促进积极应对方式的形成.
Objective:To explore the effect of narrative nursing based on caring and support on the improvement of psychological resilience and self-management in patients with secondary tuberculosis.Methods:Sixty-eight patients with secondary tuberculosis admitted to our hospital from January 2023 to January 2025 were selected as research subjects.According to the method of care,they were divided into study group(n=34,receiving narrative nursing based on caring and support)and control group(n=34,receiving routine care).Psychological resilience[assessed by Chinese version of Connor-Davidson resilience scale(CD-RISC)],self-management[assessed by chronic disease self-management scale(CDSMS)]and coping styles[assessed by medical coping modes questionnaire(MCMQ)]were compared between the two groups.Results:After nursing,the scores of CD-RISC and CDSMS in both groups increased,with the scores in study group being higher than those in control group,and the differences were statistically significant(P<0.05).Compared with control group,patients in study group demonstrated higher scores on the face dimension and lower scores on the avoidance and yield dimensions of the MCMQ after the nursing care,and the differences were statistically significant(P<0.05).Conclusion:Narrative nursing based on caring and support can significantly improve the psychological resilience of patients with secondary tuberculosis,enhance self-management ability,and promote the formation of positive coping styles.
