摘要
目的:探讨2型糖尿病肾病患者血清β2-微球蛋白(Beta-2-microglobulin,β2-MG)、尿酸(Uric Acid,UA)水平变化及与肾功能的相关性。方法:选取2022年1月至2024年5月期间本院收治2型糖尿病肾病患者55例作为观察组。根据肾功能下降程度的不同,将观察组患者分为正常组15例、轻度组20例、中度组12例、重度组8例。同时,选取同期于本院行体检的健康者55例作为对照组。检测并比较观察组与对照组血清因子(β2-MG、UA、血肌酐(Creatinine,CREA)、尿素氮(Urea)、胱抑素C(Cystatin C,CysC)水平),比较观察组不同肾功能患者的血清因子,分析观察组不同肾功能患者β2-MG、UA与肾功能相关性。结果:观察组的血清因子β2-MG、UA、CREA、Urea、CysC水平均显著高于对照组(P<0.05)。重度组的血清因子β2-MG、UA、CREA、Urea、CysC水平均显著高于正常组、轻度组、中度组;中度组的血清因子均显著高于正常组、轻度组;轻度组的血清因子均显著高于正常组(P<0.05)。正常组β2-MG、UA水平与CREA(r=0.323、0.254,P<0.05)、Urea(r=0.284、0.234,P<0.05)、CysC(r=0.309、0.216,P<0.05)水平正相关;轻度组β2-MG、UA水平与CREA(r=0.443、0.376,P<0.05)、Urea(r=0.468、0.341,P<0.05)、CysC(r=0.417、0.326,P<0.05)水平正相关;中度组β2-MG、UA水平与CREA(r=0.598、0.488,P<0.05)、Urea(r=0.546、0.446,P<0.05)、CysC(r=0.541、0.411,P<0.05)水平正相关;重度组β2-MG、UA水平与CREA(r=0.761、0.592,P<0.05)、Urea(r=0.697、0.543,P<0.05)、CysC(r=0.643、0.512,P<0.05)水平正相关。结论:2型糖尿病肾病患者血清β2-MG和UA水平显著升高,且与血清CREA、Urea、CysC水平呈正相关。血清β2-MG和UA水平可作为评估2型糖尿病肾病患者肾功能损害程度的重要指标。
Objective:To investigate the changes of serumβ2-microglobulin(Β2-MG)and uric acid(UA)levels in patients with type 2 diabetic nephropathy and their correlation with renal function.Methods:Fifty-five patients with type 2 diabetic nephropathy admitted to our hospital from January 2022 to May 2024 were selected as the observation group.According to the degree of renal function decline,the patients in the observation group were divided into normal group(15 cases),mild group(20 cases),moderate group(12 cases)and severe group(8 cases).At the same time,55 healthy people who underwent physical examination in our hospital during the same period were selected as the control group.The levels of serum factors(β2-MG,UA,serum creatinine(CREA),urea nitrogen(Urea),cystatin C(CysC))in the observation group and the control group were detected and compared.The serum factors of patients with different renal functions in the observation group were compared,and the correlation betweenΒ2-MG,UA and renal function in patients with different renal functions in the observation group was analyzed.Results:The levels of serum factorsβ2-MG,UA,CREA,Urea and CysC in the observation group were significantly higher than those in the control group(P<0.05).The levels of serum factorsβ2-MG,UA,CREA,Urea and CysC in the severe group were significantly higher than those in the normal group,the mild group and the moderate group.The serum factors in the moderate group were significantly higher than those in the normal group and the mild group.The serum factors in the mild group were significantly higher than those in the normal group(P<0.05).The levels ofβ2-MG and UA in the normal group were positively correlated with the levels of CREA(r=0.323,0.254,P<0.05),Urea(r=0.284,0.234,P<0.05)and CysC(r=0.309,0.216,P<0.05).The levels ofβ2-MG and UA in the mild group were positively correlated with the levels of CREA(r=0.443,0.376,P<0.05),Urea(r=0.468,0.341,P<0.05)and CysC(r=0.417,0.326,P<0.05).The levels ofβ2-MG and UA in the moderate group were positively correlated with the levels of CREA(r=0.598,0.488,P<0.05),Urea(r=0.546,0.446,P<0.05)and CysC(r=0.541,0.411,P<0.05).The levels ofβ2-MG and UA in the severe group were positively correlated with the levels of CREA(r=0.761,0.592,P<0.05),Urea(r=0.697,0.543,P<0.05)and CysC(r=0.643,0.512,P<0.05).Conclusion:The levels of serumβ2-MG and UA in patients with type 2 diabetic nephropathy were significantly increased,and were positively correlated with serum CREA,Urea and CysC levels.Serumβ2-MG and UA levels can be used as important indicators to evaluate the degree of renal impairment in patients with type 2 diabetic nephropathy.
