妊娠糖尿病孕妇产前糖化血红蛋白水平与新生儿体质量、血糖水平的相关性分析

摘要 目的:分析妊娠糖尿病(GDM)孕妇产前糖化血红蛋白(HbA_(1c))水平与新生儿体质量、血糖水平的相关性。方法:回顾性分析2023年1月—2024年1月于海安市妇幼保健计划生育服务中心分娩的120例GDM孕妇的临床资料,从医院电子病历管理系统中提取并整理GDM孕妇、新生儿的相关资料,根据GDM孕妇HbA_(1c)水平分为HbA_(1c)<6%组(101例)和HbA_(1c)≥6%组(19例)。比较两组GDM孕妇、新生儿相关资料,分析GDM孕妇产前HbA_(1c)水平与新生儿体质量、血糖水平的相关性。结果:两组GDM孕妇分娩前体质量指数、空腹血糖、口服葡萄糖耐量试验1 h血糖、口服葡萄糖耐量试验2 h血糖及新生儿出生体质量、初始血糖水平、大于胎龄儿情况、低血糖情况对比,差异有统计学意义(P<0.05)。Spearman相关性分析结果显示,GDM孕妇产前HbA_(1c)水平与新生儿大于胎龄儿、低血糖发生率呈正相关(r_(s)=0.392,P<0.001;r_(s)=0.357,P<0.001)。结论:GDM孕妇产前HbA_(1c)水平与新生儿大于胎龄儿、低血糖发生率呈正相关,提示GDM孕妇产前HbA_(1c)水平可作为评估新生儿不良结局风险的重要依据。 Objective:To analyze the correlation between the prenatal glycated hemoglobin(HbA_(1c))level of pregnant women with gestational diabetes mellitus(GDM)and the neonatal body weight and blood glucose level.Methods:A retrospective analysis was conducted on the clinical data of one hundred and twenty pregnant women with gestational diabetes mellitus(GDM)who delivered at Haian Maternal and Child Health Care and Family Planning Service Center from January 2023 to January 2024.Relevant data of GDM pregnant women and newborns were extracted and organized from the hospital's electronic medical record management system.Pregnant women with GDM were divided into the HbA_(1c)<6%group(101 cases)and the HbA_(1c)≥6%group(19 cases)based on their HbA_(1c) levels.The relevant data of pregnant women and newborns with gestational diabetes mellitus(GDM)in the two groups were compared,and the correlation between the prenatal HbA_(1c) level of GDM pregnant women and the body weight and blood glucose level of newborns was analyzed.Results:There were statistically significant differences in the pre-delivery body mass index,fasting blood glucose,1-hour blood glucose of the oral glucose tolerance test,2-hour blood glucose of the oral glucose tolerance test,neonatal birth weight,initial blood glucose level,large for gestational age infants,and hypoglycemia between the two groups of GDM pregnant women(P<0.05).The results of Spearman correlation analysis showed that the prenatal HbA_(1c) level of pregnant women with GDM was positively correlated with the incidence of large for gestational age newborns and hypoglycemia(r_(s)=0.392,P<0.001;r_(s)=0.357,P<0.001).Conclusion:The prenatal HbA_(1c) level of pregnant women with GDM is positively correlated with the incidence of large for gestational age newborns and hypoglycemia,suggesting that the prenatal HbA_(1c) level of GDM pregnant women can be used as an important basis for assessing the risk of adver_(s)e outcomes in newborns.
