摘要
目的:探究维持性血液透析(MHD)病人心理灵活性的现况及潜在剖面分类,分析其相关影响因素,为临床开展靶向干预提供依据。方法:采用方便抽样法选取2021年7月—2024年7月徐州医科大学第二附属医院肾内科收治且行MHD治疗的150例慢性肾衰竭病人作为调查群体。采用一般资料调查表、中文版接纳与行动问卷第2版(AAQ-Ⅱ)、医学应对方式问卷(MCMQ)、领悟社会支持量表(PSSS)进行调查。对病人心理灵活性进行潜在剖面分析,采用多元Logistic回归进一步分析影响因素。结果:MHD病人AAQ-Ⅱ条目均分为(5.02±0.89)分。MHD病人心理灵活性存在3个潜在类别,即低灵活性组(28.00%)、中等灵活性组(44.67%)和高灵活性组(27.33%)。受教育程度、家庭年收入、MCMQ中的面对维度与屈服维度、领悟社会支持是MHD病人心理灵活性不同类别的影响因素(P<0.05)。结论:MHD病人心理灵活性处于中等水平,且群体之间存在异质性。医护人员应有效识别不同心理灵活性水平的MHD病人,根据病人的特征进行针对性干预。
Objective:To explore the current situation and potential characteristics of mental flexibility among maintenance hemodialysis(MHD)patients,and analyze its related influencing factors,thereby providing a reference for clinical intervention practices.Methods:A total of 150 patients with chronic renal failure who received MHD treatment in the Department of Nephrology,the Second Affiliated Hospital of Xuzhou Medical University from July 2021 to July 2024 were selected as the investigation group by convenient sampling method.General Information Questionnaire,Chinese version of Acceptance and Action Questionnaire⁃2nd Edition(AAQ⁃Ⅱ),Medical Coping Modes Questionnaire(MCMQ)and Perceived Social Support Scale(PSSS)were used to investigate the problem.The potential profile of patients'psychological flexibility was analyzed,and the influencing factors were further analyzed by multiple Logistic regression.Results:The AAQ⁃Ⅱscores of MHD patients were(5.02±0.89).There were three potential categories of mental flexibility in MHD patients:low flexibility group(28.00%),medium flexibility group(44.67%)and high flexibility group(27.33%).Education level,family annual income,the face dimension and yield dimension in MCMQ,and perceived social support were the influencing factors for different categories of mental flexibility in MHD patients.Conclusion:The mental flexibility of MHD patients is at the medium level,and there is heterogeneity among groups.Medical staff should effectively identify MHD patients with different levels of mental flexibility and conduct targeted interventions according to the characteristics of the patients.
