摘要
唐代孙思邈所著《千金要方》集唐代以前医学之精华而成,尤其重视对妇人的治疗。《千金要方》妇科篇涵盖经、带、胎、产等疾病,本文重点论述其妇科病中对于崩漏的治疗。通过对《千金要方·崩中漏下》篇中52首治崩方剂用药选择及用药剂量进行整理与统计分析,总结孙思邈治疗妇人崩漏的规律与经验。归纳出孙思邈治疗崩漏的学术思想,其病机多为冲任虚寒、阳气不固,或阴虚血热,寒热错杂,又或是瘀血阻滞;治法重在升固阳气、调补冲任,寒热同调,阴阳兼顾,养血活血等;治则为先活血、止血、再养血,重视补气;并阐述其对于煎服法及病后重调养的特色要求。通过学习孙思邈对于崩漏的诊治特点以及方药特色,以期为中医对于崩漏的临床治疗提供参考。
Qian Jin Yao Fang by Sun Simiao in the Tang Dynasty is a collection of the essence of traditional Chinese medicine before the Tang Dynasty,with special emphasis on the treatment of gynecological diseases,and its section for gynecology covering diseases related to menstruation,leukorrhea,fetus,delivery and so on.This paper focuses on discussing the treatment ideas on metrorrhagia and metrostaxis in the book.Through the sorting and statistical analysis of 52 prescriptions and the drug dosages for treating the disease,the study means to summarize the laws and experience of Sun Simiao in this regard.It is concluded that the pathogeneses of the disease mostly feature deficient cold of thoroughfare and conception channels,non-consolidation of yang qi,or yin deficiency and blood heat,intermingled cold and heat,or static blood obstruction and stagnation,that its treatment focuses on ascending and reinforcing yang qi,regulating the Chong and Ren meridians,harmonizing cold and heat,giving consideration to yin and yang balance,nourishing blood and promoting blood circulation,etc.,that its treatment principle is first to promote blood circulation,stop bleeding,and then nourish blood,and pay attention to invigorating qi.The paper meanwhile elaborates on its characteristic requirements for decocting approaches and recuperation after illness.By studying the characteristics of Sun Simiao's diagnosis and treatment of the disease and the characteristics of his prescriptions and medication,the study is expected to provide reference for the clinical treatment of metrorrhagia and metrorrhagia in TCM.
