摘要
目的:探讨前哨淋巴结活检(SLNB)与腋窝淋巴结清扫(ALND)对乳腺癌患者上肢功能的影响。方法:选取新沂市人民医院2020年1月—2023年12月收治的乳腺癌患者89例,按照手术方式分为SLNB组(n=43)和ALND组(n=46)。比较两组手术干预效果。结果:SLNB组手术时间、术中出血量及术后引流量低于ALND组(P<0.001);SLNB组上肢水肿、肩关节活动受限及上肢感觉异常发生率低于ALND组(P=0.007);术后12个月,SLNB组上臂周径小于ALND组(P<0.001),肩关节活动度、肩关节最大外展角度大于ALND组(P<0.001);SLNB组在术后1周、1个月及3个月的视觉模拟评分法评分低于ALND组(P<0.001)。结论:SLNB在保证乳腺癌腋窝分期与治疗效果的前提下,可减少术后损伤和并发症,改善上肢功能,缓解术后疼痛。
Objective:To explore the influence of sentinel lymph node biopsy(SLNB)and axillary lymph node dissection(ALND)on upper limb function in breast cancer patients.Methods:A total of 89 patients with breast cancer who were admitted to Xinyi People's Hospital from January 2020 to December 2023 were selected.According to the surgical methods,the patients were divided into SLNB group(n=43)and ALND group(n=46).The effects of surgical intervention were compared between the two groups.Results:The operation time,intraoperative blood loss,and postoperative drainage volume in the SLNB group were lower than those in the ALND group(P<0.001).The incidences of upper limb edema,restricted shoulder joint motion and upper limb paresthesia in the SLNB group were lower than those in the ALND group(P=0.007).At 12 months after surgery,the upper arm circumference in the SLNB group was smaller than that in the ALND group(P<0.001),and the shoulder range of motion and maximum abduction angle of shoulder joint in the SLNB group were larger than those in the ALND group(P<0.001).The visual analogue scale scores in the SLNB group were lower than those in the ALND group at 1 week,1 month and 3 months after surgery(P<0.001).Conclusion:On the premise of ensuring axillary staging and therapeutic effect of breast cancer,SLNB can reduce postoperative injury and complications,improve upper limb function and relieve postoperative pain to a certain extent.
