摘要
目的:探究出血性脑卒中(HS)患者肠内营养启动时间现况调查状及其成功启动的影响因素。方法:选取2023年7月—2025年1月福建医科大学附属龙岩第一医院收治的223例HS患者为研究对象,根据肠内营养是否成功启动将患者分为两组:启动成功组(n=196)和启动失败组(n=27)。收集患者基础信息,运用logistic回归模型分析对HS患者肠内营养成功启动的影响因素进行分析。结果:在本研究223例HS患者中,196例(87.89%)患者肠内营养成功启动,27例(12.11%)患者肠内营养启动失败。两组在性别、身体质量、营养筛查评分(NRS)、基础疾病(高血压、糖尿病)方面比较,差异无统计学意义(P>0.05)。在年龄、是否机械通气、置管方式、是否添加膳食纤维、是否服用促胃动力药物及肠内营养启动时间方面,两组组间比较,差异有统计学意义(P<0.05)。且经logistic多因素回归分析示,年龄、未进行机械通气、添加膳食纤维、服用促胃动力药物、肠内营养启动时间均是HS患者肠内营养成功启动的影响因素,差异有统计学意义(P<0.05)。结论:大部分HS患者肠内营养能成功启动,年龄、未进行机械通气、添加膳食纤维、服用促胃动力药物、肠内营养启动时间均为HS患者肠内营养成功启动的重要影响因素。
Objective:To explore the current situation of enteral nutrition initiation time in patients with hemorrhagic stroke(HS)and the influencing factors of successful initiation.Method:A total of 223 HS patients admitted to the Longyan First Hospital Affiliated to Fujian Medical University from July 2023 to January 2025 were selected as the research subjects.The patients were divided into two groups based on whether enteral nutrition was successfully initiated:the successful initiation group(n=196)and the failed initiation group(n=27).The basic information of patients was collected,and the logistic regression model was used to analyze the influencing factors of the successful initiation of enteral nutrition in HS patients.Result:Among the 223 HS patients in this study,enteral nutrition was successfully initiated in 196 patients(87.89%),while it failed in 27 patients(12.11%).There were no statistically significant differences between the two groups in terms of gender,body weight,nutritional screening score(NRS),and underlying diseases(hypertension,diabetes)(P>0.05).There were statistically significant differences between the two groups in terms of age,whether mechanical ventilation was used,catheterization method,whether dietary fiber was added,whether gastric motility drugs were taken,and the initiation time of enteral nutrition(P<0.05).Moreover,logistic multivariate regression analysis showed that age,no mechanical ventilation,addition of dietary fiber,use of gastric motility drugs,and the initiation time of enteral nutrition were all influencing factors for the successful initiation of enteral nutrition in HS patients,and the differences were statistically significant(P<0.05).Conclusion:Enteral nutrition can be successfully initiated in most HS patients.Age,absence of mechanical ventilation,addition of dietary fiber,use of gastric motility drugs,and the initiation time of enteral nutrition are all important influencing factors for the successful initiation of enteral nutrition in HS patients.
