摘要
目的:探究柴胡清肝汤联合多西他赛、卡铂对乳腺癌患者细胞免疫功能及血清微小核糖核酸-125(miR-125)、微小核糖核酸-21(miR-21)的影响。方法:前瞻性选取2021年4月~2024年5月期间某院收治的60例Ⅲ期乳腺癌患者,采用简单随机化分组方法分为化疗组和联合组,每组30例。化疗组给予多西他赛注射液、注射用卡铂治疗,联合组在化疗组基础上给予柴胡清肝汤治疗。比较两组临床疗效、细胞免疫功能(CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+))、血清miR-125、miR-21水平、中医证候积分及不良反应发生情况。结果:治疗6个周期后,联合组疾病控制率(83.33%)高于化疗组(60.00%,P<0.05);CD3^(+)、CD4^(+)和CD4^(+)/CD8^(+)均高于化疗组(P<0.05)。两组血清miR-125均升高,且联合组高于化疗组(P<0.05)。血清miR-21和乳房肿块、情志异常、胸胁胀满和脘腹胀闷评分均降低,且联合组低于化疗组(P<0.05)。两组骨髓抑制、过敏反应、胃肠道不适和肝肾功能异常的发生率比较均无统计学差异(P>0.05)。结论:柴胡清肝汤联合多西他赛、卡铂可有效提升乳腺癌患者的生存获益,改善中医证候,调控miR-125、miR-21表达,纠正免疫紊乱,且不会增加不良反应的发生风险。
Objective:To investigate the effects of Chaihu Qinggan decoction combined with docetaxel and carboplatin on cellular immune function and serum microRNA-125(miR-125)/microRNA-21(miR-21)in patients with breast cancer.Methods:A total of 60 patients with stageⅢbreast cancer treated in a hospital between April 2021 and May 2024 were prospectively selected and assigned to the chemotherapy group and combination group using a simple randomization method,with 30 patients in each group.The chemotherapy group was treated with docetaxel injection and carboplatin for injection,while the combination group was treated with Chaihu Qinggan decoction in addition to the treatment given in the chemotherapy group.The clinical efficacy,cellular immune function(CD3^(+),CD4^(+),CD4^(+)/CD8^(+)),serum levels of miR-125 and miR-21,traditional Chinese medicine(TCM)syndrome scores and adverse reactions were compared between the two groups.Results:After 6 treatment cycles,the disease control rate in the combination group(83.33%)was higher than that in the chemotherapy group(60.00%,P<0.05).CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)were higher in the combination group than in the chemotherapy group(P<0.05).The serum miR-125 was increased in both groups,with the combination group showing higher levels than the chemotherapy group(P<0.05).The serum miR-21 and scores for breast masses,emotional abnormalities,fullness in the chest and hypochondrium,and abdominal distension were decreased in both groups,with the combination group showing lower values than the chemotherapy group(P<0.05).There were no statistically significant differences in the incidence of myelosuppression,allergic reactions,gastrointestinal discomfort and abnormal liver and kidney function between the two groups(P>0.05).Conclusion:Chaihu Qinggan decoction combined with docetaxel and carboplatin can effectively improve survival benefits in patients with breast cancer,alleviating TCM syndromes,regulating miR-125 and miR-21 expression,correcting immune dysfunction,without increasing the risk of adverse reactions.
