摘要
目的研究米非司酮联合甲氨蝶呤与腹腔镜手术治疗异位妊娠的临床疗效,分析其对患者hCG水平及妊娠结局的影响。方法选取2022年7月-2024年7月在我院诊治的60例异位妊娠患者为研究对象,采用随机数字表法分为对照组和观察组,各组30例。对照组采用米非司酮联合甲氨蝶呤治疗,观察组采用腹腔镜手术治疗,对比两组治疗成功率、血清hCG水平、临床症状改善时间、卵巢功能、妊娠结局及并发症发生率。结果观察组治疗成功率(100.00%)高于对照组(90.00%)(P<0.05);两组血清hCG均低于治疗前,且观察组低于对照组(P<0.05);观察组腹痛消失、阴道出血消失、血清hCG恢复正常、月经恢复正常时间均短于对照组(P<0.05);两组卵巢功能评分均低于治疗前,且观察组低于对照组(P<0.05);随访1年,观察组宫内妊娠率(63.33%)高于对照组(43.33%)(P<0.05);观察组并发症发生率(6.67%)低于对照组(16.67%)(P<0.05)。结论米非司酮联合甲氨蝶呤与腹腔镜手术治疗异位妊娠均可取得一定的疗效,但是相对而言腹腔镜手术治疗效果更确切,可提高治疗成功率,更大幅度降低hCG水平,快速减轻患者临床症状,改善患者卵巢功能和妊娠结局,且并发症发生率低,有效性和可行性更佳。
Objective To study the clinical efficacy of mifepristone combined with methotrexate and laparoscopic surgery in the treatment of ectopic pregnancy,and to analyze its effect on hCG level and pregnancy outcome.Methods Sixty patients with ectopic pregnancy diagnosed and treated in our hospital from July 2022 to July 2024 were selected as the research objects.They were divided into control group and observation group by random number table method,with 30 patients in each group.The control group was treated with mifepristone combined with methotrexate,and the observation group was treated with laparoscopic surgery.The treatment success rate,serum hCG level,clinical symptom improvement time,ovarian function,pregnancy outcome and complication rate were compared between the two groups.Results The success rate of treatment in the observation group(100.00%)was higher than that in the control group(90.00%)(P<0.05).The serum hCG of the two groups was lower than that before treatment,and the observation group was lower than the control group(P<0.05).The time of disappearance of abdominal pain,disappearance of vaginal bleeding,recovery of serum hCG and recovery of menstruation in the observation group were shorter than those in the control group(P<0.05).The ovarian function scores of the two groups were lower than those before treatment,and the observation group was lower than the control group(P<0.05).After 1 year of follow-up,the intrauterine pregnancy rate of the observation group(63.33%)was higher than that of the control group(43.33%)(P<0.05).The incidence of complications in the observation group(6.67%)was lower than that in the control group(16.67%)(P<0.05).Conclusion Mifepristone combined with methotrexate and laparoscopic surgery in the treatment of ectopic pregnancy can achieve a certain effect,but the effect of laparoscopic surgery is more accurate.It can improve the success rate of treatment,greatly reduce the level of hCG,quickly reduce the clinical symptoms of patients,improve the ovarian function and pregnancy outcome of patients,and the incidence of complications is low,the effectiveness and feasibility are better.
