摘要
目的 探讨输卵管远端造口术联合活血利水通络方治疗输卵管积水的临床疗效。方法选取2021年9月至2023年9月河北省迁安市中医医院收治的150例输卵管积水患者作为研究对象,进行回顾性研究。按照治疗方案的不同分为对照组(采用输卵管远端造口术治疗,72例)和观察组(采用输卵管远端造口术联合活血利水通络方治疗,78例)。比较两组治疗前后卵巢储备功能、子宫内膜容受性,比较两组治疗后12个月妊娠结局和未妊娠输卵管积水复发情况。结果 治疗后两组抗米勒管激素(AMH)、雌二醇(E_(2))水平显著高于治疗前,且观察组高于对照组(P<0.05);治疗后两组卵泡刺激素(FSH)、子宫动脉阻力指数(RI)、子宫动脉搏动指数(PI)显著低于治疗前,且观察组低于对照组(P<0.05)。治疗后两组子宫内膜厚度显著大于治疗前,且观察组大于对照组(P<0.05)。观察组宫内妊娠率显著高于对照组(P<0.05),异位妊娠率、未妊娠率和未妊娠输卵管积水复发率显著低于对照组(P<0.05);两组辅助生殖技术妊娠率比较,差异无统计学意义(P > 0.05)。结论 输卵管远端造口术联合活血利水通络方治疗输卵管积水患者的临床效果显著,可有效改善患者的卵巢储备功能和子宫内膜容受性,提高宫内妊娠率,降低复发率。
Objective To explore the clinical efficacy of distal salpingostomy combined with Huoxue Lishui Tongluo Prescription in the treatment of hydrosalpinx.Methods A retrospective research was conducted and the 150 patients with hydrosalpinx admitted to Qian′an Traditional Chinese Medicine Hospital of Hebei Province from September 2021 to September 2023 were selected as the research subjects.According to different treatment plans,they were divided into the control group(treated with distal salpingostomy,72 cases)and the observation group(treated with distal salpingostomy combined with Huoxue Lishui Tongluo Prescription,78 cases).The ovarian reserve function and endometrial receptivity between two groups before and after treatment were compared,the pregnancy outcomes of two groups after 12 months of treatment and the recurrent hydrosalpinx in non-pregnant women were compared.Results After treatment,the levels of anti-Müllerian hormone(AMH)and estradiol(E_(2))in the two groups were significantly higher than before treatment,and the observation group were higher than the control group(P<0.05).After treatment,the levels of follicle stimulating hormone(FSH),uterine artery resistance index(RI),and uterine artery pulsatility index(PI)in the two groups were significantly lower than before treatment,and the observation group were lower than the control group(P<0.05).After treatment,the endometrial thickness in the two groups was significantly greater than before treatment,and the observation group was greater than the control group(P<0.05).The intrauterine pregnancy rate in the observation group was significantly higher than that in the control group(P<0.05),the rates of ectopic pregnancy,non-pregnancy,and recurrence of non-pregnancy hydrosalpinx were significantly lower than those in the control group(P<0.05);there was no statistically significant difference in pregnancy rates between the two groups of assisted reproductive technologies(P>0.05).Conclusions The clinical effect of distal salpingostomy combined with Huoxue Lishui Tongluo Prescription in the treatment of hydrosalpinx patients is significant.It can effectively improve the ovarian reserve function and endometrial receptivity of patients,increase the intrauterine pregnancy rate,and reduce the recurrence rate.
