摘要
目的:观察基于筋柔骨正理论运用柔筋微调法治疗儿童寰枢关节半脱位的疗效,以及对患儿寰齿间距、疼痛及颈椎功能的影响。方法:选取2023年1月—2024年10月杭州市丁桥医院收治的60例寰枢关节半脱位患儿,按随机数字表法分为试验组和对照组各30例。对照组给予肌肉放松疗法及常规颈椎牵引治疗,试验组给予常规颈椎牵引及柔筋微调法治疗。2组均治疗2周。评估2组临床疗效,观察2组治疗前后寰齿前间隙、寰齿侧间距差值、疼痛程度[数字分级评分法(NRS)评分]、颈椎旋转活动度及颈椎功能障碍程度[颈椎功能障碍指数(NDI)评分]的变化。结果:治疗2周后,试验组总有效率93.33%(28/30),高于对照组70.00%(21/30)(P<0.05)。2组寰齿前间隙、寰齿侧间距差值均较治疗前缩小(P<0.05),试验组寰齿前间隙、寰齿侧间距差值均小于对照组(P<0.05)。2组NRS、NDI评分均较治疗前降低(P<0.05),试验组NRS、NDI评分均低于对照组(P<0.05)。2组颈椎旋转活动度均较治疗前增大(P<0.05),试验组颈椎旋转活动度大于对照组(P<0.05)。结论:基于筋柔骨正理论运用柔筋微调法治疗儿童寰枢关节半脱位疗效确切,患儿的疼痛症状及体征均明显好转,颈椎功能恢复良好。
Objective:To observe the curative effect of Roujin Weitiao therapy on atlantoaxial subluxation in children based on the theory of tendon-soothing and bone-setting.Methods:A total of 60 children with atlantoaxial subluxation admitted to Hangzhou Dingqiao Hospital from January 2023 to October 2024 were selected and divided into the trial group and the control group according to the random number table method,with 30 cases in each group.The control group was treated with muscle relaxation and conventional cervical traction,and the trial group was treated with conventional cervical traction and Roujin Weitiao therapy.Both groups were treated for two weeks.The clinical effects in the two groups were evaluated,and the changes in atlanto-dental interval,variance of lateral atlanto-dental interval,pain degree[scores of Numerical Rating Scale(NRS)],and cervical range of motion and neck disability degree[scores of Neck Disability Index(NDI)]before and after treatment were observed between the two groups.Results:After two weeks of treatment,the total effective rate was 93.33%(28/30)in the trial group,higher than that of 70.00%(21/30)in the control group(P<0.05).The atlanto-dental interval and variance of lateral atlanto-dental interval in the two groups were reduced when compared with those before treatment(P<0.05),and the atlanto-dental interval and variance of lateral atlanto-dental interval in the trial group were smaller than those in the control group(P<0.05).The NRS and NDI scores in the two groups were decreased when compared with those before treatment(P<0.05),and the NRS and NDI scores in the trial group were lower than those in the control group(P<0.05).The cervical range of motion in the two groups was increased when compared with that before treatment(P<0.05),and the cervical range of motion in the trial group was higher than that in the control group(P<0.05).Conclusion:Roujin Weitiao therapy has a definite curative effect on atlantoaxial subluxation in children based on the theory of tendon-soothing and bone-setting,which can significantly improve pain and body signs and well recover the cervical spine function.
