摘要
目的:观察参芪地黄温肾汤治疗脾肾阳虚型慢性肾小球肾炎临床疗效及对炎症因子水平的影响。方法:选取濮阳市中医医院2022年10月—2024年11月期间收治的106例脾肾阳虚型慢性肾小球肾炎患者为研究对象,采用随机数字表法分为基础组和研究组各53例。基础组给予阿魏酸哌嗪片等常规治疗,研究组在基础组的基础上加用参芪地黄温肾汤治疗。比较2组临床疗效及不良反应,以及2组治疗前后中医证候评分、肾功能[24 h尿蛋白定量(24hUpro)、血尿素氮(BUN)、血肌酐(SCr)、肾小球滤过率(GRF)]、炎症因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-17(IL-17)、C-反应蛋白(CRP)]水平变化。结果:治疗后,研究组总有效率为96.23%(51/53),基础组为81.13%(43/53),组间比较,差异有统计学意义(P<0.05)。治疗后,2组中医证候评分均较治疗前降低(P<0.05),且研究组中医证候评分低于基础组(P<0.05)。治疗后,2组24hUpro、BUN、SCr水平均较治疗前降低(P<0.05),GFR水平较治疗前均升高(P<0.05);且研究组24hUpro、BUN、SCr水平均低于基础组,GFR水平高于基础组(P<0.05)。治疗后,2组TNF-α、IL-6、IL-17、CRP水平均较治疗前降低(P<0.05),且研究组TNF-α、IL-6、IL-17、CRP水平均低于基础组(P<0.05)。2组患者均未出现明显不良反应。结论:参芪地黄温肾汤治疗能够减轻脾肾阳虚型慢性肾小球肾炎患者症状,改善肾功能及炎症反应,提高疗效,且安全性高。
Objective:To observe the clinical effect of Shenqi Dihuang Wenshen Decoction on chronic glomerulonephritis of spleen-kidney yang deficiency type and its effect on inflammatory factor levels.Methods:A total of 106 cases of patients with chronic glomerulonephritis of spleen-kidney yang deficiency type admitted to Chinese Medicine Hospital of Puyang from October 2022 to November 2024 were selected and divided into the basic group and the study group according to the random number table method,with 53 cases in each group.The basic group was given conventional treatment including Piperazine Ferulate Tablets,and the study group was additionally treated with Shenqi Dihuang Wenshen Decoction based on the treatment of the basic group.Changes in traditional Chinese medicine syndrome scores,kidney function[24-hour urine protein(24hUpro),blood urea nitrogen(BUN),serum creatinine(SCr),and glomerular filtration rate(GFR)],and levels of inflammatory factors[tumor necrosis factorα(TNFα),interleukin-6(IL-6),interleukin-17(IL-17),and C-reactive protein(CRP)]before and after treatment as well as clinical effects and adverse reactions were compared between the two groups.Results:After treatment,the total effective rate was 96.23%(51/53)in the study group and 81.13%(43/53)in the basic group,the difference being significant(P<0.05).After treatment,traditional Chinese medicine syndrome scores in the two groups were decreased when compared with those before treatment(P<0.05),and the score in the study group was lower than that in the basic group(P<0.05).The levels of 24hUpro,BUN,and SCr in the two groups were reduced when compared with those before treatment(P<0.05),and GFR levels were increased when compared with those before treatment(P<0.05);the levels of 24hUpro,BUN,and SCr in the study group were lower than those in the basic group,and the GFR level was higher than that in the basic group(P<0.05).Levels of TNF-α,IL-6,IL-17,and CRP in the two groups were decreased when compared with those before treatment(P<0.05),with lower levels in the study group than in the basic group(P<0.05).No significant adverse reactions were observed in either group.Conclusion:Shenqi Dihuang Wenshen Decoction can alleviate symptoms,improve kidney function and inflammation reactions,enhance therapeutic efficacy,and demonstrate high safety in treating chronic glomerulonephritis of spleen kidney yang deficiency type.
