摘要
“成渝双城经济圈”建设为区域医疗协同提供战略机遇,区县级医院作为连接优质医疗与基层需求的关键节点,却普遍面临“夹心层”困境。本研究以重庆市大足区人民医院眼科为案例,分析其发展现状,剖析学科定位、人才结构、诊疗体系、科研能力等问题,并结合与成都中医学院附属银海医院的医联体实践,探讨发展机遇与挑战。研究发现,该院眼科虽有三级医院专科基础,但受地理与资源虹吸影响,难以吸引疑难重症患者,对基层辐射不足。据此,从明确差异化学科定位、构建跨区域人才联合培养机制、深化成渝医疗协同、完善眼健康分级诊疗、加强科研创新与科普宣传五维度优化路径,配套政策、资金、质量监督等保障措施,为该科室破局提供方案,也为区县级医院学科建设提供可复制经验,助力区域卫生健康一体化与“健康中国”战略落地。
The construction of the“Chengdu Chongqing Dual City Economic Circle”provides strategic opportunities for regional medical collaboration.County level hospitals,as key nodes connecting high-quality medical care with grassroots needs,generally face the dilemma of the“sandwich layer”.This study takes the ophthalmology department of Dazu District People’s Hospital in Chongqing as a case study to analyze its current development status,discipline positioning,talent structure,diagnosis and treatment system,scientific research capabilities,and other issues.Combined with the practice of medical consortium with Yinhai Hospital affiliated to Chengdu University of Traditional Chinese Medicine,it explores development opportunities and challenges.Research has found that although the ophthalmology department has a specialized foundation in tertiary hospitals,it is difficult to attract difficult and severe patients due to geographical and resource barriers,resulting in insufficient radiation to the grassroots level.Based on this,an optimization path is proposed from five dimensions:clarifying the positioning of differentiated departments,establishing a cross regional talent joint training mechanism,deepening Chengdu Chongqing medical collaboration,improving eye health grading diagnosis and treatment,strengthening scientific research innovation and popular science propaganda,and supporting policies,funds,quality supervision and other guarantee measures.This provides a plan for the department to break through the situation,as well as replicable experience for the discipline construction of district and county-level hospitals,and helps to promote the integration of regional health and the implementation of the“Healthy China”strategy.
