摘要
目的全面系统地评估住院医师的药学相关知识与技能水平,明确当前药学素养现状,为规范化培训阶段药学教育的优化提供建议。方法采用文献调研、专家访谈和问卷调查相结合的方法,对江苏省正在接受培训的住院医师进行药学素养认知自评和客观测试评估。江苏省13个市的29家培训单位中的504位住院医师参与问卷调查。结果经过筛选,保留471份有效问卷。住院医师药学素养自评得分为(3.80±0.60)分,客观测试得分为(16.67±6.53)分,正确率为53.77%,提示其药学知识掌握仍有提升空间。亚组分析显示,接受培训时间较长、通过执业医师考试的住院医师药学素养自评得分较高(P<0.05),但客观测试得分组间差异无统计学意义。同时,团队配备临床药师、医院开展药学相关培训及知晓合理用药制度均显著提高了住院医师药学素养自评和客观测试得分(P<0.05)。结论部分住院医师高估自身药学素养,其药学知识储备和合理用药能力仍需提升。强化规范化培训阶段的药学教育,对培养高质量的临床医师具有重要的战略意义。
Objective To systematically evaluate residents’pharmacy-related knowledge and skills,clarify the current status of their pharmacy competency,and provide evidence for optimizing pharmacy education during residency.Methods A mixed-method approach,including literature review,expert interviews,and questionnaire,was used to assess both self-perceived and objectively tested pharmacy competency among residents in hospital of Jiangsu province.Totally of 504 residents from 29 training institutions across 13 cities responded to the survey.Results After data screening,471 valid questionnaires were included in the final analysis.The mean self-assessment score of pharmacy competency was 3.80±0.60,while the objective test score was 16.67±6.53,with an accuracy rate of 53.77%,indicating room for improvement in pharmacy knowledge.Subgroup analysis showed that residents with longer years of training and those who had passed the medical licensing examination had much higher self-assessment scores(P<0.05),although no obvious differences were observed in the objective test scores between groups.Additionally,the presence of clinical pharmacists in the team,hospital-based pharmacy training,and awareness of rational drug use policies were all associated with markedly higher self-assessment and objective test scores(P<0.05).Conclusion Some residents overestimate their pharmacy competency,and their pharmaceutical knowledge and rational prescribing skills need improvement.Strengthening pharmacy education during residency is important for cultivating high-quality clinicians.
