MLCK、PCNA在结肠癌组织表达与患者转移和预后中的影响分析

摘要 目的 研究肌球蛋白轻链激酶(MLCK)和增殖细胞核抗原(PCNA)在结肠癌组织中的表达及与结肠癌患者转移和预后的影响。方法 选取2019年1月至2023年1月行手术切除的结肠癌患者100例,收集结肠癌组织和相对应的癌旁组织(距肿瘤边缘>5 cm),其中MLCK和PCNA表达。结果 100例中,MLCK阳性表达患者83例。而癌旁组织中MLCK阳性表达组织47例,癌组织的MLCK阳性表达率明显高于癌旁组织(P<0.05)。结肠癌组织中,PCNA阳性表达率为89.00%,而癌旁组织PCNA的阳性表达率为17.00%,癌组织的PCNA阳性表达率明显高于癌旁组织(P<0.05)。有远处转移结肠癌患者的MLCK阳性表达率高于无远处转移患者(P<0.05),有淋巴结转移的结肠癌患者的MLCK阳性表达率高于无淋巴结转移患者(P<0.05),TNM分期为Ⅲ期和Ⅳ期患者的MLCK阳性表达率高于Ⅰ期和Ⅱ期患者(P<0.05),中高分化结肠癌组织中MLCK阳性表达率高于低分化患者(P<0.05)。有远处转移结肠癌患者的PCNA阳性表达率高于无远处转移患者(P<0.05),有淋巴结转移的结肠癌患者的PCNA阳性表达率高于无淋巴结转移患者(P<0.05),TNM分期为Ⅲ期和Ⅳ期患者的PCNA阳性表达率高于Ⅰ期和Ⅱ期患者(P<0.05)。MLCK蛋白阳性表达患者的生存时间短于MLCK蛋白阴性表达患者(P<0.05),PCNA蛋白阳性表达患者的生存时间短于PCNA蛋白阴性表达患者(P<0.05)。单因素分析显示患者MLCK蛋白表达、PCNA蛋白表达、肿瘤分化程度、血管淋巴管浸润、淋巴结转移、TNM分期是影响患者预后的独立危险因素(P<0.05),进一步进行多因素分析发现,患者MLCK蛋白表达、PCNA蛋白表达、肿瘤分化程度、血管淋巴管浸润、淋巴结转移、TNM分期是影响结肠癌患者预后的独立危险因素(P<0.05)。结论 结肠癌组织中MLCK和PCNA蛋白表达水平升高,远处转移结肠癌患者MLCK和PCNA阳性表达率较高,其表达水平与患者预后存在相关性;为临床对结肠癌患者的远处转移和预后寻找新的治疗药物和靶点提供研究基础。 Objective To study the expressions of myosin light chain kinase(MLCK)and proliferating cell nuclear antigen(PCNA)in colon cancer tissues and their correlation with metastasis and prognosis.Methods A total of 100 patients with colon cancer who underwent surgical resection in our hospital from January 2019 to January 2023 were included.Colon cancer tissue and corresponding adjacent tissue(>5 cm from the tumor edge)of all patients were collected.The expressions of MLCK and PCNA in colon cancer and adjacent normal tissues were detected.Results Among the 100 colon cancer tissues,83 patients had positive expression of MLCK,with the positive expression rate of 83.00%;while in the adjacent tissues,47 patients had MLCK positive expression,with the positive expression rate of 47.00%.The positive expression rate of MLCK in cancer tissues was significantly higher than that of adjacent tissues(P<0.05).In colon cancer tissue,there were 89 cases of PCNA-positive tissue,with the positive expression rate of 89.00%;there were 17 cases of PCNA-positive adjacent normal tissue,with a positive expression rate of 17.00%.The positive expression rate of PCNA in cancer tissue was significantly higher than that of adjacent tissues(P<0.05).The positive expression rate of MLCK in colon cancer patients with distant metastasis was significantly higher than that in patients without distant metastasis(P<0.05).The positive expression rate of MLCK in colon cancer patients with lymph node metastasis was significantly higher than that in patients without lymph node metastasis(P<0.05).The positive expression rate of MLCK in patients with stageⅢ-Ⅳwas significantly higher than that in patients with stageⅠ-Ⅱ(P<0.05).The positive expression rate of PCNA in colon cancer patients with distant metastasis was significantly higher than that in patients without distant metastasis(P<0.05).The positive expression rate of PCNA in colon cancer patients with lymph node metastasis was significantly higher than that in patients without lymph node metastasis(P<0.05).The positive expression rate of PCNA in patients with stageⅢ-Ⅳwas significantly higher than that in patients with stageⅠ-Ⅱ(P<0.05).The survival time of patients with positive expression of MLCK was significantly shorter than that of patients with negative expression of MLCK(P<0.05),and the survival time of patients with positive expression of PCNA was significantly shorter than that of patients with negative expression of PCNA(P<0.05).Univariate analysis showed that the protein expression of MLCK,the protein expression of PCNA,the degree of tumor differentiation,angiolymphatic infiltration,lymph node metastasis and tumor,node,metastasis(TNM)staging were significantly associated with the prognosis of colon cancer(P<0.05).Multivariate analysis further showed that he protein expression of MLCK,the protein expression of PCNA,tumor differentiation,angiolymphatic invasion,lymph node metastasis,and TNM staging were independent risk factors for the prognosis of colon cancer patients(P<0.05).Conclusion The protein expressions of MLCK and PCNA are upregulated in colon cancer tissue,and the positive expression rates of MLCK and PCNA are higher in colon cancer patients with distant metastasis.Their expression levels are related to the prognosis of colon cancer.This study provides a research basis for finding new therapeutic drugs and targets for distant metastasis and prognosis of colon cancer.
