摘要
目的系统评价中药外敷治疗输液性静脉炎的临床疗效。方法检索中国知网、万方数据知识服务平台、中国生物医学文献服务系统、维普网、Pub Med等数据库中有关中药外敷治疗输液性静脉炎的随机对照试验(randomized controlled trial,RCT)。采用Cochrane偏倚风险评估工具评价纳入研究的方法学质量,使用RevMan 5.4.1软件进行Meta分析,并应用试验序贯分析(trial sequential analysis,TSA)和证据评价与推荐意见分级、制定和评价方法学(grading of recommendations assessment,development,and evaluation,GRADE)进行证据质量评级。结果最终纳入15项RCT,共1435例患者。Meta分析结果显示中药外敷可显著提高临床总有效率,缩短疼痛缓解时间、疼痛消失时间和红肿消退时间,并降低视觉模拟评分法评分(P<0.05)。TSA结果表明临床总有效率的结论具有统计学可靠性。GRADE评价显示总体证据等级偏低。结论中药外敷治疗输液性静脉炎总体疗效优于常规西药外用,但鉴于纳入研究的证据质量偏低,此结论尚需更多高质量、大样本研究证实。
Objective To systematically evaluate the clinical efficacy of Chinese medicine external application for treatment of infusion phlebitis.Methods Search for randomized controlled trials(RCT)on the Chinese medicine external application for the treatment of infusion phlebitis in databases such as CNKI,Wanfang Data Knowledge Service Platform,SinoMed,VIP,and PubMed.The methodological quality of the included studies was evaluated using the Cochrane risk of bias assessment tool.Meta-analysis was conducted using RevMan 5.4.1 software,and the quality of evidence was rated using trial sequential analysis(TSA)and grading of recommendations assessment,development,and evaluation(GRADE).Results A total of 15 RCT with 1435 patients were included.The Meta-analysis showed that Chinese medicine external application significantly increased the overall clinical effective rate and shortened the time to pain relief,pain disappearance,and redness and swelling regression,and reduced the score of visual analogue scale(P<0.05).The TSA results indicated that the conclusion of the total clinical effective rate was statistically reliable.The GRADE assessment showed that the overall level of evidence was relatively low.Conclusion The Chinese medicine external application in the treatment of infusion phlebitis is generally superior to the conventional external application of Western medicine.However,given the low quality of evidence in the included studies,this conclusion still needs to be confirmed by more high-quality and large-sample studies.
