摘要
目的:系统评价升陷汤加减联合常规西药治疗慢性心力衰竭的临床疗效。方法:计算机检索中国知网数据库、万方数据库、维普数据库、中国生物医学文献数据库及PubMed、the Cochrane Library、EMbase中收录的相关文献,使用RevMan 5.4.1软件对3篇及以上有相同结局指标的临床研究的相关数据进行分析。结果:本研究最终纳入22项研究,共涉及病例1681例。Meta分析结果显示,与对照组相比,升陷汤加减联合常规西药治疗可显著提高疗效[临床疗效:OR=3.16,95%CI(2.19,4.56),P<0.00001;中医证候疗效:OR=4.64,95%CI(2.57,8.41),P<0.00001;心功能分级疗效:OR=2.83,95%CI(1.78,4.50),P<0.0001],并显著改善中医证候评分[MD=–2.90,95%CI(–3.65,–2.14),P<0.00001]、左室射血分数(Left Ventricular Ejection Fraction,LVEF)[MD=4.04,95%CI(2.41,5.66),P<0.00001]。结论:升陷汤加减联合常规西药治疗慢性心力衰竭疗效确切,今后还需要更多大样本、高质量随机对照临床研究进行进一步的验证分析,以提供更高质量的证据及评价。
Objective:To systematically evaluate the clinical efficacy of the Shengxian decoction(升陷汤)plus conventional Western drugs in the treatment of chronic heart failure.Methods:The relevant studies included in CNKI,Wanfang database,VIP database,CBM,PubMed,the Cochrane Library,and EMbase were retrieved by computer.The relevant data of three or more clinical studies with the same outcome indicators was analyzed by the RevMan 5.4.1 software.Results:Twenty-two studies were ultimately included,involving 1681 cases.The Meta-analysis results showed that compared with the control group,the Shengxian decoction plus conventional Western drugs significantly improved the efficacy[Clinical efficacy:OR=3.16,95%CI(2.19,4.56),P<0.00001,therapeutic effect of TCM syndrome amelioration:OR=4.64,95%CI(2.57,8.41),P<0.00001;therapeutic effect of cardiac function grading amelioration:OR=2.83,95%CI(1.78,4.50),P<0.0001],and significantly ameliorated TCM syndrome score[MD=-2.90,95%CI(-3.65,-2.14),P<0.00001]and LVEF[MD=4.04,95%CI(2.41,5.66),P<0.00001].Conclusion:The efficacy of the Shengxian decoction plus conventional Western drugs in the treatment of chronic heart failure is accurate,and more large-sample,high-quality randomized controlled clinical studies are needed for further validation and analysis in the future to provide higher-quality evidence and evaluation.
