针刀干预对膝骨关节炎兔软骨细胞TFR1、FPN蛋白表达的影响

Effects of Acupotomy Intervention on Protein Expressions of TFR1 and FPN in Chondrocytes of KOA Rabbits

摘要 目的:观察针刀对于膝骨关节炎(KOA)兔软骨细胞铁离子代谢水平的影响,探讨其治疗KOA的作用机制。方法:24只新西兰雄性兔按随机数字表法分为空白组、模型组、针刀组和电针组。应用过伸位固定制动法(改良版Videman法)制动6周建立KOA模型。治疗干预针刀组和电针组3周,采用改良版Lequesne MG指数评分和膝关节被动活动度(PROM)评估行为学,运用HE染色法观察软骨细胞组织形态,透射电镜观察线粒体结构,运用Western blot技术检测软骨细胞中TFR1、FPN蛋白的含量。结果:与空白组比较,模型组兔Lequesne MG评分显著升高,PROM显著降低,差异具有统计学意义(P<0.01);软骨细胞排列紊乱,炎性细胞浸润明显,线粒体皱缩、线粒体嵴减少甚至消失;模型组中TFR1的表达明显升高,FPN的表达明显抑制,差异具有统计学意义(P<0.01)。与模型组比较,针刀组、电针组Lequesne MG评分显著降低,PROM显著升高,差异具有统计学意义(P<0.01);软骨细胞排列较整齐,炎性细胞浸润减少,线粒体数量较多,结构较正常,线粒体嵴增多;针刀组和电针组中TFR1蛋白表达明显下调,差异具有统计学意义(P<0.01或P<0.05),而FPN蛋白表达显著升高,差异具有统计学意义(P<0.01)。与电针组比较,针刀组Lequesne MG评分下降,差异具有统计学意义(P<0.05),PROM增加,差异具有统计学意义(P<0.01);TFR1蛋白表达降低,差异具有统计学意义(P<0.05),FPN蛋白表达明显升高,差异具有统计学意义(P<0.01)。结论:针刀干预可调控TRF1、FPN蛋白表达,提高铁离子代谢水平,达到保护软骨的目的,其治疗效果优于电针。 Objective:To investigate the effects of acupotomy on iron ion metabolism in chondrocytes of rabbits with knee osteoarthritis(KOA),and to explore its underlying therapeutic mechanisms.Methods:A total of 24 male New Zealand rabbits were randomly assigned into the blank control group,the model group,the acupotomy group,and the electro-acupuncture(EA)group.The KOA model was established using a 6-week immobilization protocol in an overextension position(modified Videman method).Treatment interventions for the acupotomy group and the EA group lasted 3 weeks.Behavioral assessments were performed using the Lequesne MG index and passive range of motion(PROM)of the knee joint.Histological morphology of chondrocytes was examined with HE staining,and mitochondrial ultrastructure was observed via transmission electron microscopy.Protein expressions of TFR1 and FPN in chondrocytes were determined using Western blot analysis.Results:Compared with the blank control group,rabbits in the model group exhibited significantly higher Lequesne MG scores and markedly reduced PROM of the knee joint(P<0.01);chondrocyte arrangement was disorganized,inflammatory cell infiltration was prominent,and mitochondrial changes included shrinkage,reduced cristae,or even disappearance;the TFR1 expression was significantly up-regulated,whereas the FPN expression was markedly suppressed in the model group(P<0.01).Compared with the model group,both the acupotomy group and the EA group showed significantly lower Lequesne MG scores and significantly higher PROM values(P<0.01);chondrocyte arrangement was more regular,inflammatory cell infiltration was reduced,and mitochondria were more numerous with relatively normal structure and increased cristae;the protein expression of TFR1 was significantly down-regulated(P<0.01,P<0.05),whereas the protein expression of FPN was markedly up-regulated in the acupotomy group and the EA group(P<0.01).Compared with the EA group,the acupotomy group showed a greater reduction in Lequesne MG scores(P<0.05)and a more pronounced increase in PROM(P<0.01);in addition,TFR1 protein expression was lower(P<0.05)and FPN protein expression was significantly higher(P<0.01)in the acupotomy group.Conclusion:Acupotomy intervention can regulate the protein expressions of TFR1 and FPN and enhance iron ion metabolism,thereby protecting articular cartilage.Its therapeutic efficacy is superior to that of EA.
