摘要
目的:观察电针对膝骨性关节炎(KOA)大鼠股四头肌生肌转录因子、炎性因子的影响,探讨其起效机制。方法:将30只雄性SD大鼠随机分成空白组、模型组及电针组,每组10只。采用膝关节制动法制备KOA模型。电针组取KOA大鼠患侧“足三里”“阳陵泉”,1次/2 d,共干预14次。干预后,测量各组大鼠Lequesne MG评分、湿重及肌湿重体质量比值;采用步态分析技术测量各组大鼠最大爪印面积、最大压强以及摆动速度;HE染色法观察各组大鼠股四头肌形态,免疫组织化学染色法与荧光定量PCR法检测各组大鼠右侧股四头肌中配对成肌分化抗原(MyoD)、肌细胞生成素(MyoG)、Ⅰ型肌球蛋白重链(MyHC1)以及β-连环蛋白(β-catenin)mRNA表达;ELISA法检测各组大鼠股四头肌肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)及白细胞介素-6(IL-6)含量。结果:与模型组比较,电针能降低Lequesne MG评分(P<0.01),差异有统计学意义,增加KOA大鼠右后肢最大爪印面积、最大压强和摆动速度(P<0.01),增加股四头肌湿重、湿重体质量比值(P<0.01),改善KOA大鼠膝关节软骨和股四头肌形态结构,上调KOA大鼠股四头肌MyoD、MyoG、MyHC1及β-catenin阳性表达,差异均具有统计学意义(P<0.05或P<0.01),增加KOA大鼠股四头肌MyoD、MyoG、MyHC1及β-catenin mRNA表达,差异均具有统计学意义(P<0.05或P<0.01),降低股四头肌炎症因子TNF-α、IL-1β及IL-6含量差异均具有统计学意义(P<0.05或P<0.01)。结论:电针“足三里”“阳陵泉”能够改善KOA大鼠关节炎指数、调节异常步态,其作用机制可能与上调生肌标志物MyoD、MyoG、MyHC1及β-catenin表达、降低股四头肌炎症和促进骨骼肌再生修复相关。
Objective:To investigate the effects of electro-acupuncture(EA)on myogenic transcription factors and inflammatory cytokines in the quadriceps muscle of rats with knee osteoarthritis(KOA),and to explore its underlying action mechanisms.Methods:30 male SD rats were randomly divided into the blank group,the model group and the EA group,with 10 rats in each group.The KOA model was prepared by knee joint immobilization.The EA group was intervened by electro-needling affected side Zusanli(ST36)and Yanglingquan(GB34),once every other day for 14 times.After the intervention,the Lequesne MG score was assessed in each group of rats.Wet muscle weight and the muscle wet weight-to-body weight ratio were measured.Gait analysis technology was used to evaluate the maximum paw print area,peak pressure,and swing speed.HE staining was performed to observe quadriceps muscle morphology.Immunohistochemistry and quantitative real-time PCR(qRT-PCR)were used to detect the mRNA and protein expressions of MyoD,MyoG,MyHC1,and β-catenin in the right quadriceps muscle.The contents of TNF-α,IL-1β,and IL-6 in the quadriceps muscle were measured using ELISA.Results:The Lequesne MG score was decreased(P<0.01),the maximum paw print area,maximum compression and swing speed of the right hind limb were increased(P<0.01),the wet muscle weight and the muscle wet weight-to-body weight ratio of the quadriceps muscle were also increased(P<0.01),the morphology and structure of the knee joint cartilage and quadriceps muscle were improved,the positive protein and mRNA expressions of MyoD,MyoG,MyHC1,and β-catenin in quadriceps muscle were increased(P<0.05,P<0.01),and the inflammatory factors of TNF-α,IL-1β and IL-6 in quadriceps femoris muscle were reduced in the EA group compared to those in the model group(P<0.05,P<0.01).Conclusion:Electro-needling ST36 and GB34 can improve arthritis index and regulate abnormal gait in KOA rats,and its action mechanism may be related to the up-regulation of MyoD,MyoG,MyHC1,and β-catenin,the reduction of inflammation in quadriceps femoris muscle and the promotion of skeletal muscle repair.
