摘要
目的探究二甲双胍联合吡格列酮对2型糖尿病(type 2 diabetes mellitus,T2DM)合并冠状动脉硬化性心脏病(简称冠心病)经皮冠状动脉介入治疗(percutaneous coronary intervention,PCI)术后患者糖脂代谢的影响,为临床治疗方案优化提供依据。方法非随机方便选取2022年1月—2025年7月于天津市人民医院接受PCI治疗的60例T2DM合并冠心病患者,根据不同治疗方法分为对照组、观察组,每组30例。对照组接受二甲双胍治疗,观察组应用吡格列酮联合二甲双胍治疗。比较两组血糖、血脂水平及不良反应发生情况。结果治疗后,观察组空腹血糖、餐后2 h血糖、糖化血红蛋白水平、总胆固醇、甘油三酯、低密度脂蛋白、高密度脂蛋白水平均优于对照组,差异均有统计学意义(P均<0.05)。不良反应组间对比,差异无统计学意义(P>0.05)。结论二甲双胍联合吡格列酮可更有效改善T2DM合并冠心病PCI术后患者糖脂代谢,且安全性良好。
Objective To explore the effect of metformin combined with pioglitazone on glucose and lipid metabolism in patients with type 2 diabetes mellitus(T2DM)complicated with coronary atherosclerotic heart disease(coronary heart disease)after percutaneous coronary intervention(PCI),and to provide a basis for clinical treatment optimization.Methods Non-randomized convenient selection of 60 patients with T2DM complicated with coronary heart disease who received PCI in Tianjin People's Hospital from January 2022 to July 2025,and divided by different treatment methods.The control group(30 cases)was treated with metformin,and the observation group(30 cases)was treated with pioglitazone combined with metformin.The blood glucose,blood lipid levels and adverse reactions were compared between the two groups.Results After treatment,the levels of fasting plasma glucose,2-hour postprandial plasma glucose and glycated hemoglobin A1c,total cholesterol,triglyceride,low density lipoprotein and high density lipoprotein of the observation group were better than those in the control group,the differences were statistically significant(all P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion For patients with T2DM complicated with coronary heart disease after PCI,metformin combined with pioglitazone can improve glucose and lipid metabolism with good safety.
