摘要
目的分析糖尿病(diabetes mellitus,DM)病程与空腹C肽(fasting C-peptide,FCP)水平对成人隐匿性自身免疫糖尿病(latent autoimmune diabetes,LADA)患者合并糖尿病周围神经病变(diabetic peripheral neuropathy,DPN)的影响。方法回顾性选取2022年1月—2024年12月贵航贵阳医院收治的92例LADA患者的临床资料,根据是否合并DPN分为DPN组(41例)与非DPN组(51例),比较两组一般资料、糖代谢指标、FCP和餐后2 h C肽(2-hour postprandial C-peptide,2 hPCP)水平,并分析LADA并发DPN的影响因素。结果DPN组年龄、病程分布、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)、糖化血红蛋白、胰岛素抵抗指数(homeostatic model assessment of insulin resistance,HOMA-IR)高于非DPN组,FCP、2 hPCP、胰岛β细胞功能指数(homeostatic model assessment of isletβcell function index,HOMA-β)低于非DPN组,差异均有统计学意义(P均<0.05)。二元Logistic回归分析显示,DM病程>10年、FCP、2 hPCP、HOMA-β和HOMA-IR是LADA并发DPN的影响因素(P均<0.05)。结论DM病程、2 hPCP、FCP与LADA发生DPN密切相关,病程越长、胰岛素抵抗越重、FCP与2 hPCP水平越低发生DPN风险越高。
Objective To analyze the effect of diabetes mellitus(DM)course and fasting C-peptide(FCP)level on diabetic peripheral neuropathy(DPN)in adult patients with latent autoimmune diabetes(LADA).Methods The clinical data of 92 patients with LADA admitted to Guizhou Airlines Guiyang Hospital from January 2022 to December 2024 were retrospectively selected.According to whether DPN was combined,they were divided into DPN group(41 cases)and non-DPN group(51 cases).The clinical data,glucose metabolism indexes,FCP and postprandial 2-hour C-peptide(2 hPCP)levels were compared between the two groups,and the influencing factors of LADA complicated with DPN were analyzed.Results The age,course of disease distribution,low-density lipoprotein cholesterol(LDL-C),glycated hemoglobin A1c,and homeostatic model assessment of insulin resistance(HOMA-IR)in the DPN group were higher than those in the non-DPN group,FCP,2 hPCP and homeostatic model assessment of islet β cell function index(HOMA-β)were lower in DPN group than those in non-DPN group,and the differences were statistically significant(all P<0.05).Binary Logistic regression analysis showed that the DM course>10 years,FCP,2 hPCP,HOMA-β and HOMA-IR were the influencing factors of LADA complicated with DPN(all P<0.05).Conclusion The course of DM,2 hPCP and FCP are closely related to the occurrence of DPN in LADA.The longer the course of DM,the heavier the insulin resistance and the lower the FCP and 2 hPCP level,the higher the risk of DPN.
