摘要
目的评价PDCA循环管理法在控制住院患者抗菌药物合理使用的应用效果。方法回顾性收集某医疗机构2023年3~12月住院患者抗菌药物使用数据作为基线,2024年1~2月整理数据资料并找出问题,运用PDCA循环管理法分析抗菌药物使用不合理的原因,并拟定对策,建立合理应用抗菌药物的相关措施,2024年1~9月为PDCA循环管理活动的干预期,2024年10月为循环干预后,对比实施PDCA循环前后住院患者抗菌药物使用强度的趋势。结果实施PDCA循环管理法干预后,该医疗机构住院患者抗菌药物使用强度(DDDs)于2024年10月下降至28.8DDDs,住院患者抗菌药物使用率下降至37.12%,I类切口手术抗菌药物预防使用率下降至24.27%,均达到PDCA管理目标。结论科学实施PDCA循环管理工具可有效改善医疗机构住院患者抗菌药物不合理使用情况,提高抗菌药物合理用药水平。
Objective To evaluate the effectiveness of the Plan-Do-Check-Action(PDCA)cycle management method in controlling the rational use of antimicrobial drugs among hospitalized patients.Methods Retrospective data on antimicrobial drug use among hospitalized patients at a certain medical institution from March to December 2023 were collected as baseline data.From January to February 2024,data were organized,issues were identified,and the PDCA cycle management method was applied to analyze the causes of inappropriate antibiotic use,formulate countermeasures,and establish measures for rational antibiotic use.January to September 2024 constituted the intervention period for the PDCA cycle management activities.October 2024 as post-cycle intervention period,the trends in antibiotic use intensity among hospitalized patients before and after implementing the PDCA cycle were compared.Results After implementing the PDCA cycle management method intervention,the antibiotic use intensity(Defined Daily Dose,DDDs)among inpatients at the medical institution decreased to 28.8 DDDs by October 2024,the antibiotic use rate among inpatients decreased to 37.12%,and the rate of prophylactic antibiotic use for Class I surgical incisions decreased to 24.27%,all meeting the PDCA management targets.Conclusion The scientific implementation of the PDCA cycle management tool can effectively improve the inappropriate use of antimicrobial agents among hospitalized patients in medical institutions and enhance the rational use of antimicrobial agents.
